What Does This Ultrasound Report During 38 Weeks Of Pregnancy Indicate?

Question: Hello Dr. - I am XXXXXXX and wanted your advice to my query whether to go for normal delivery of C section.

I am 34 years years old and have a 5.5 years old son. I was married in 2009. MY first delivery was C section because baby had cord round the neck and I was presuurized by everybody to go for C section though I always wanted normal delivery.

I had a missed abortion around 11 months back and we need to go D&C for the same.

After that I concived and is willing for normal delivery. Currently I am in 38 week and everything seems to be fine with regard to my blood reports , hameglobin , baby movements etc.

I am attaching my latest ultra sound report. There the scar thickness in 3.2 therefore existing doctor is suggesting that there is risk to go for normal delivery and suggesting to go for C section. I spoke to other doctor and she said normal delivery could be done.

I am totally confused as I want a normal delivery.

Please suggest and let me know if you need addtional information from my end.

Apart from cord round neck, was there any other reason for a C section ?
3. If you do not mind sharing the details, which doctors have you consulted and in which city ? A lot depends on professional competence and integrity of the doctors .
4. Are you willing to undergo calculated risks ( possible risk of uterine rupture, possible risks of fetal distress etc ) while attempting a normal delivery this time round ?

Thanks for your reply. Please find below the required information:

1. Height: 5feet7inches Weight: Current Weight 80kg ; Pre Pregnancy 64kg
2. Last time C section was done at 39 weeks. It was an elective C secton without any labor pain. Baby weight was 3.5 kgs. There was no other reason for C section as far as our knowledge and this was in XXXXXXX in 2009
3. Both doctors whom I have consulted now are based in XXXXXXX and are renowned doctors
4. We know about calculated risk but not sure the extent of the risk and probability of success associated with the same. Not sure whom to believe

Please let me know if you need more information. Current baby weight as per the latest ultrasound report is 2.8kgs.

Need advice what should we do in this situation. We know about the risk of normal delivery after C section but need to know how much are the chances of success.

Hello again.
What we are discussing here is VBAC - vaginal birth after cesarean section.
Normally success rates of VBAC vary from 50 - 80 % depending on a lot of factors.
1. Maternal age more than 30 reduces chances of successful VBAC.
2. Scar thickness less than 4 mm increases risk of complications such as uterine rupture during labour.
3. Weight more than 80 kg reduces chances of VBAC.
Current baby weight is fine.
In my opinion, you should go ahead with knowledge of calculated risk.
Chances of success of VBAC in your case is more than 50 % even considering the adverse factors such as age, thinness of previous scar etc.
Factors in your favour are - less baby weight, non recurrent indication of C section, more than 18 months since first C section.
The doctor whom you choose should herself / himself believe in VBAC.
I can suggest few names if you are willing to conduct the delivery in XXXXXXX
For XXXXXXX you have to research such a person.
In good , experienced hands and with provision of vigilant , round the clock obstetric care, there is no reason why you should not attempt VBAC.
Attempt at least, if not succeed.
But without attempting, there would be no success !

Thanks for your advice doctor. Probably last set of questions. In this scenario , what is your advice if we try to induce labour in our case. Also, what if we dont get labour pains; how much time should we wait to opt for a C section?

If I understood you correctly, you are advicing to try for a normal delivery and in case it is not going well then anyway we can get C section done. I hope there is no risk to the baby as per the process.

In the last, any further advice which you would like to give in order to prepare ourself for normal delivery.

I already told you - there is ALWAYS a risk to the mother and child when attempting normal delivery in a case of previous C section.
And I explained the percentages to you also.
That is why it is a calculated risk.
YOu go for the trial of normal delivery considering all factors and understanding the risks - hence the term ' calculated ' risk.
BUt that should not deter you from trying as long as your doctor is confident and willing to participate in the process with full caution and measures.
The advice is to consult a doctor renowned for VBAC and its successful and experienced practise and then go ahead with the delivery in a labour suite equipped with round the clock emergency facilities.
Indulge in regular exercise, prevent excessive weight gain.
I hope you have enrolled in antenatal classes that teach good pelvic floor exercises.
Most important is the mental preparation for the fact that you are undertaking a calculated risk, but chances of success are more than 50 %.
