Brief Answer:
Possible causes mentioned down

Detailed Answer:
Oh, Great!
I requested and submitted the previous answer and could see this posted.

Seen the report.
The 4 mm cyst can not cause pain unless it has a pedicle/ stalk around which it can rotate.
There is no evidence on ultrasonography and color doppler of any infection.
Infection will cause continuous and severe pain with fever and chills and usually the symptoms of urinary tract infection will be present.

Some other causes of pain in one side of the testicle/ scrotum are:
- Development of hernia which stretches the nerves supplying the testicle.
- As you have rightly mentioned about your trial to walk
(( I have vertigo so I walk funny and I started trying to walk straighter the last two weeks . (14 years of vertigo). My stomach or mid section has been getting tighter because I am trying to walk the correct way ( using my core ))
The stretched muscles can cause the entrapment of the nerves supplying the testis.

Medicines may help you relieve the pain, please continue .
4 mm cyst may not cause the pain.
Infection is less likely. (Get the blood and urine checked) and ultrasonography of the abdomen also must be done to see for Genito-urinary system.

Please give feedback .
