What Does This Ultrasound Report Indicate?

Question: Hi,
This is my wife 2nd pregnancy and her scan shows 21 weeks and 2 days prwgnancy, We did level 2 scan today and all feutus organs are shown to be normal but doppler scan is showing diastolic notch in both arteries. Is this a serious problem plz confirm and let me know for precautions.

we have a history of premature 8 months delivery of our first baby due to diastolic reversal flow and baby expired after three days on ventilator.

Hi dear,
I have gone through your question and understand your concerns.
Notch in diastolic flow is suggestive of decreased blood flow to the fetus, which can lead to fetal growth restriction , decreased fluid around the fetus or sudden intrauterine death.
It can be various diseases like hypertension in pregnancy, autoimmune disease or antiphospholipid antibody syndrome.
Since your wife had a similar history in last pregnancy, the diagnosis of antiphospholipid antibody syndrome or autoimmune disease like systemic lupus erythematosus are more likely.
She should get regular ultrasound weekly to assess the fetal well being as well as Doppler.
As soon as there is some derangement like absent diastolic flow or reversal of diastolic flow, immediate delivery should be done.

Hi Doctor,
Is it possible that it can get away in next few weeks as i read somewhere that diastolic notch upto 23 weeks is not to worry about. Moreover my wife has been advised tl take Maintain 500 (Inj Once a week) and Delispirin 75 (BT). will that help to overcome this problem..moreover if you can suggest me any hospital with a high technology to fight this problem preferably around XXXXXXX

Hi dear,
Diastolic notch at 22-23 weeks is unlikely to go away.Maintaine injection is progesterone and has no role this problem.
Aspirin do helps in increasing the blood flow to the fetus.
You can visit any tertiary level hospital or medical college in XXXXXXX for specialized care.
