Brief Answer:
Could be due to Bladder Outlet Obstruction

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that your Ultrasound Scanning has revealed to have mild Hydronephrosis and pelviectasis on left side and getting UTI very frequently.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Is the hydronephrosis on both sides or only on left side ?
2) Upload report of your Ultrasound Scanning .

Bilateral hydronephrosis with frequent episodes of UTI is mostly due to incomplete emptying of bladder resulting from bladder outlet obstruction and needs to be confirmed

Consult qualified Urologist for clinical examination and get following basic
tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Endoscopic Examination of bladder (Cystoscopy)

In he mean while start taking broad spectrum antibiotics like Cefixime along with urinary antiseptic like Nitrofurantoin twice daily and urine alkaliser thrice daily.Later on switch on to appropriate antibiotics as per culture report.

Further treatment will depend upon result of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.

You have not uploaded the report of your scan in its complete and proper format as regards paranchymal thickness ,cortico medullary differentiation etc .It is difficult to comment based on findings that you have narrated and need to undergo few more tests

I would suggest you to consult Urologist for detail evaluation to assess the cause for atrophy of Rt Kidney and functions of both the kidneys by doing following tests
1) Renal function tests
2) Intra Venous Pyelography (I.V.P)
3) DTPA scan
