Brief Answer:
Chemotherapy treatment with new drug

Detailed Answer:
HI

USG showing dilated echoic fluid pelvic cavity indicate her disease has progressed and there is ascites in her abdomen.

She needs to get her ascetic fluid tapped followed by a CT scan of whole abdomen to see the current disease status.

For pain she can take tablet paracetamol 1 gm thrice daily after food.

Swelling will settle down once the fluid is removed from her abdomen by tapping.

For vomiting she can take metoclopramide tablets before food twice daily with doctors advise.

I would advise chemotherapy for her.

Her Ca 125 has increased because her disease has progressed
