What Does This Widal Test Result Indicate?

Question: Today is 6/09/2015. My son age 7yrs and weight 19kg is suffering with high fever since 26/08/2015, later it was found in test its typhoid (typhi-O) with 1:160 value. Widal test again done on 4/09/2015 now its typhoid with typhi-O (1:80) and typhi-H (1:80). He sometimes utter in sleep but it was also present before fever also.
Sometimes fever rises to 103.3'F.
Treatment is under experienced doctor retired from Govt. Hospital at New XXXXXXX The days when Glucose drip was given fever was less and only once in 24 hrs.
What should I do? Its been longtime with high fevers....
I need reply now its very urgent.

This is Typhoid fever and I have a few questions for you

Detailed Answer:
Hi...I have gone through your reports. All the tests including slightly lower platelet counts are suggestive of Typhoid fever or Enteric fever.

Usually Typhoid after starting on the correct antibiotics on the correct dosage might take - 5-14 days to settle completely. The dose you have used is lower dose (if what you have mentioned is accurate).

I have a few questions for you -

1. How is the fever on 28th August and now?
2. has the height of fever and frequency come down?
3. How is your son's appetite on 28th and now?
4. Is there any diarrhea or rash or abdominal distension?
5. How is his activity in-between two fever spikes?
6. Are any other family members suffering form fever?
7. Is there any wound like or insect bite like ulcer in the flexures of the body?
8. Is he having any puffiness of eyes or face?
9. Have blood cultures tests been done before starting antibiotics?

Please get back with answers and I will be glad to guide you further.

1. on 28th it was near to 103.5F but its lower now, but today it touched 103.3F.
Fever comes with shivering.
2. height of fever if relatively down and frequency is less on the days glucose drip given. now its twice in 24 hrs atleast. sometimes thrice.
3. earliar he dint want to have anything not even water, now when there is no fever he asks for food and digests it, we provide him bread with tea, liquid daal, juice, and yesterday he had small piece of wheat roti also with liquid daal.
4. He goes to toilet once a day with dark greenish very watery stool.
5. He behaves very normal between two fever strikes.
6.No one else is suffering from fever, but today his mother is complaining of body ache and sore throat and fever.
7. yes there is wound like or insect bite like ulcer in the flexures of the body on his right side of chest and small one on his right arm bisecp.
8.There is no swelling on his face.
9. These are the only test done. Medication started at the time of first test.
oh one more test was done on 29-09-2015 with no dengue, no malaria, no typhoid. Pallatelet count 1.36

My suggestion for you -

1. I feel that it is recovering typhoid.

2. The medicatoins doses needs to be adjusted - I suggest using Tablet Cefixime 200mg twice a day for 14 days.

3. I want you to upload the image of the ulcer you found on the body and if it is suggestive of Scrub typhus ulcer - we need to change the medication.

This is 100% eschar of scrub typhus - please admit him

Hi... I am assertively telling you that this is not typhoid.

THE IMAGE YOU HAVE UPLOADED IS THE CLASSICAL ULCER OF SCRUB TYPHUS.

HE NEEDS TO BE HOSPITALIZED.

THE MEDICATIONS NEEDS TO BE CHANGED.

THE DRUG OF CHOICE WILL BE - DOXYCYCLINE OR AZITHROMYCIN.

My suggestions for you -

1. Please take him to Fortis Hospital and hospitalize him.
2. I am 100% convinced that this is SCRUB TYPHUS. If the doctors there are not convinced - show them the eschar you have uploaded as an image. If still not convinced - then please call me I will talk to them (0000). This will not get better unless - the above mentioned drugs are used.
