What Does Vaginal Discharge 8 Weeks After Hysterectomy Indicate?

default
Posted on Thu, 13 Apr 2017
Question: I had a hysterectomy on July 13, 2016. When I went to see doctor after my six weeks, he put silver nitrate because I wasn't fully healed and said I should be fine after 8 weeks. I had been sore for much longer, but thought that was just part of the normal healing process. I kept having vaginal discharge, however, and thought something else was wrong, so I went to see the family practice doctor (5 months post op). She said it wasn't healed and sent me back to gyno. He said I had granulation tissue, and treated it with silver nitrate and gave me estrogen cream, and I was to come back in a couple weeks. After a couple weeks, still there was not much progress, so he treated again, and I was to come back in 6 weeks. After six weeks, it still wasn't healed, so he treated again and I was to come back in another 6 weeks (which is next week). He said if it still isn't healed they will remove the granulation tissue. It still hurts a lot, so I suspect I am in the same boat, but I'm very nervous about removing the granulation tissue. How much more sore will I be after that? will it help it finally heal? Is it normal it is getting close to a year after surgery and I'm still not healed? This sucks.

Brief Answer:
Some more inputs. have you used any other medical vaginal tablets.

Detailed Answer:
Hi
Dr. Purushottam welcomes you to HCM virtual clinic.
I have gone through your query.
I think I have understood your concern, I will try to suggest you the best possible treatment options.

1) First of all do not panic.

2)Sorry to hear about your problem of vaginal healing.
Many times after hysterectomy, healing of the top portion of vagina takes time, or sometimes there is issue of granulation tissue.
This is very delicate velvety tissue that forms near the vaginal healing portion. And is vulnerable to get infected and cause vaginal discharge.

3) I would like to know, if you are getting foul smell or vaginal itching along with discharge?
And have you used any other vaginal medicated tablets?

I hope my answer helps you.
Thanks.
Wish you good health.
With regards
Dr Purushottam Neurgaonkar

I have been using Estrace Vaginal Cream (no other creams or tablets). No foul smell, minor itching (but I am told sometimes the Estrace cream can cause some itching), but I have already been tested for any issues like infection etc and have tested fine. I am mostly concerned about how well I can expect to heal if I have the granulation tissue removed. If it didn't heal this time, how likely will granulation re-occur?

Brief Answer:
After removal, wound heals fast, rarely recurs.

Detailed Answer:
Hi there,
Thanks for appreciating the answer and reverting back.

Basically, formation of granulation tissue is one type of healing response.
Many a times if it persists, it needs to be treated.
Most of the times oral course of antibiotics, local vaginal anti fungal and anti bacterial tablets help.
If needed, granulation tissue is surgically tackled by using cold knife cautery or electrosurgical cautery. This is done via vaginal route, under anesthesia.
So please do not worry about the procedure.

Post removal of granulation tissue, healing of the wound is very fast.
Usually, once done, problem of granulation tissue rarely recurs.

I hope this answer helps you.
Thanks.
