Brief Answer:
As detailed below.

Detailed Answer:
Hi.
Thanks for your query.
Noted the history about your husband and understood your concerns.
Also noted the Medical history about diabetes, stroke and current medications.

The most probable cause of repeated spasms making him feel like he must evacuate his bowels are:

The neurological problems that have affected the bladder also involve the anal sphincter causing it to act the way he is feeling.

Local causes like ano-proctitis may be ruled out by ano-proctoscopy and sigmoidoscopy and treated if found to have any problems.

No, not Neurologists, first of all, an X-ray of the abdomen, along with a clinical evaluation by a General Surgeon or a Proctologist can help further; if fecal impaction, fecolith loaded colon is found out then soap-water enema can help to evacuate the fecoliths and may help to solve the problem he has.

I hope this answer helps your husband; please get these things done and give feedback.
Feel free to ask for further relevant queries if you feel that there is a gap of communication.
Regards
