Question : My maid detected with secondary syphilis. Undergone 10 days monocef 1 gm iv and 14 days minoz.tpha was 1:640. After year tested positive for vdrl at R1 solution and tpha 1:160. Doctors asking to take peniciline for seronegative, but assuring that he is cured. Kindly give your veiw. What is r1 dilition
, tpha value showing improvement and he is fully cured or not

Seronegativity not an indicator; azithromycin best

Detailed Answer:
Hi there XXXXXX.
I understand your concerns & empathise.
Your maid is in the clear & no further tests or treatment required.
Seronegativity is often never achieved & if there is a 2-4 fold drop in titre (as it is in your case), you may consider the treatment as being effective.
R1 dilution is a difficult concept to understand & involves an entitiy called a 'prozone phenomenon' that occurs in these serology reactions' ...do not sweat over this..it suffices that it was done & the lab appears competent.
As I said, no further treatment is required but for peace of mind, please do take a 15 day course of Azithromycin 1 gram tab taken twice/day for 15 days & close the chapter. Remember to impress upon him the virtues of safe sex & do hope his partner/s have been treated to prevent recurrences.
Good Luck & trust all your queries have been suitably addressed & he does appear well on the road to recovery if not totally cured. Cheers.
