Kidney stropy means deterioration of structure as well as function of the kidney.
Atrophy can be caused by a number of causes like chronic infection or chronic inflammation or due to back pressure changes due to obstruction of the kidney caused by stones or some mass.
The function decreases in atrophied kidneys.
The kidney function deteriorates or decreases as the atrophy sets increases.

Usually an atrophic kidney reaches end stage kidney disease.
The only treatment for atrophy kidney is renal transplant.
