Question: Can you explain a round hypoechoic lesion in the right lobe of the liver within segment V11, measuring 2.4 cm in diameter, this is a non vascular lesion. Hepatitis C was diagnosed approximately ten years ago. What is the prognosis and any treatment options for my husband who had this ultrasound that showed this lesion in September 2014
Many thanks,
XXXX

Brief Answer:
Need to rule out hepatocellular carcinoma

Detailed Answer:
Hi XXXXXXX
Thanks for writing in to us.

I have read through your query.
What I understand is that your husband got a ultrasound abdomen done for his chronic hepatitis C status.

Other than the segment VII 2.4 cm hypoechoic lesion, the report states that the liver is enlarged and measures 17.7 cm, with this the liver also appears fatty and coarse, a finding associated with chronic liver disease.

The 2.4 cm area needs to be evaluated for the possibility of Hepato Cellular Carcinoma due to of hepatitis C. This hypovascular lesion needs to be studied by 4 phase CT scan. I have come across cases where a hypovasular HCC can appear in a hepatitis C patient. If confirmed, at this stage it is 2.4 cm and surgical resection is one of the "curative" therapies available.

We need to keep in mind that the above is only a possibility and it can also be a benign lesion and that does not pose any problem.

The prognosis of a HCC is bad if it measures more than 5 cm. Apart from this chronic liver disease is classified into Child-Pugh class A to C depending on factors like bilirubin, albumin, PT INR, ascites and hepatic encephalopathy. Your husband might be in class A and that has got a good prognosis.

Hope your query is answered.
Please write back if you have any doubts.
