What Is Moderate Right Foraminal Narrowing At C5-6?

I read your question carefully and I am sorry about the symptoms you are experiencing.
The term discophytosis is not commonly used, usually we speak either of disc herniation or osteophytosis (but I see that you live in Kuwait so there may be slight differences in terminology). Anyway that report speaks of narrowing of the right foramina between the 5th and the 6th neck vertebrae.
You should know that from the cervical spinal cord come out in pairs of nerves which go to the shoulder and upper limbs on th right and left. These nerves exit the spine by passing into these canals between the vertebrae which are called foramina. If there is an issue leading to narrowing of the foramina it can be associated with nerve compression.
This can be caused either by bulging of the discs which are found between the vertebral bodies (disc herniation) or from osteophytes bony growths in the setting of rheumatic changes of the spine which progress with age.
So in your case either osteophytosis or disc herniation (or both) narrow the foramen and compress the nerve.
The only thing which doesn't really fit with that report is that you feel more symptoms on the left while the affected foramina is on the right. So there might be something else in that report to explain that, if you upload the full report might be better.
In terms of treatment, initially it is with anti-inflammatory pain killers and muscle relaxants which you seem to be taking, so your medication is correct. A cervical collar is helpful as well. Furthermore a physical therapy program must be scheduled and begun as it's the best means for a long term improvement.
If such conservative measures are not enough then local injections of steroids and anesthetics might ease the pain while the physical therapy is continued.
If none of the above works then surgery may be needed to remove the disc fragments and/or osteophytes.
I remain at your disposal for other questions.

I checked my full biochem profile and all are ok,except for my low urea and creatinine, iron,vit.d.. I lose some weight bec. I lost my appetite due to unbearable pain from shoulder.I tried to wear cervical collar this morning i feel comfortable with my spine(c5-c6)but the the problem is...its hitting my shoulder esp the base of the neck that still give me pain.

Thank you for the added information.
As you say the medication is pretty much the same for cervical and lumbar issues. You are right in saying that long term use can have its side effects, they shouldn't be used in eternity. If you do not have any known conditions and the pain is really intense though they can be taken for a couple more weeks. As for the collar I wonder what type of collar have you taken, a soft collar at the right size for you shouldn't hurt the shoulder.
For the vitamin d and iron deficiency you could use over the counter supplements, while you speak of low urea and creatinine which is actually fine, it's the increased values we fear (unless you wrote low by mistake in which case you shouldn't take the above mentioned drugs anymore, only acetaminophen for the pain).
However as I said before you shouldn't lose sight of the only long lasting measure which is physical therapy. Those medications do not deal with the spinal issue directly, they merely alleviate the symptoms, neither does the collar, physical therapy is the only means to address the spinal issue.
I hope you'll feel better soon.

Im using soft collar. I had one seesion with a physio therapist last friday and it releaves me, the pain was less now, but there is some points that still hurts, my arm movements still limited but not like before. She gave some exercise that i have to do 2x a day....But im confused one told me to put warm compress and some told me to put cold compress!

Low levels of creatinine are not common but they can be observed at times when there is a lowered muscle mass. So the weight loss may have played a role.
Yes by couple I meant 2 more weeks, regularly. But if you are pain free or only mild pain there is no reason to continue, can interrupt earlier, as I said they don't influence much the condition, only alleviate symptoms. For mild pain simple acetaminophen can suffice.
It is good you have started physiotherapy, in time it will reinforce your core muscles and diminish the load the spine has to deal with. As for the hot and cold issue, they both have been described in the literature to provide relief in some patients, success of one or the other varies among individuals, personally I recommend warmth, but the effect is modest to be honest.
I hope to have been of help.

Thank you
