Detailed Answer:

Hi,

Xray finding are very typicaly suggestive of "Osteoarthritis"of both shoulder Joints. Osteoarthritis is very common disease of the joints specially with advanced age .Unfortunately till now we dont have a very effective treatment for it howe'ver there are many ways we can manage it .
Osteoarthritis it self is caused by degeneration of the carriage between joints ( hence reduced joint space on xry).The managemnt is as follow .

1. No curative tretment is there , we just need to control the pain. mostly this is enough.
2. Paracetamol is first line tretment to Control the pain , so it can be used regularly like 1 to 2 tablet every 6 hours .
3. NSAIDS like brufen , or diclofenac sodium are effective in controlling pain , howe'ver they are not recommended for long time and only for few day to few weeks when the pain is very sever. (not to be used if peptic ulcer or renal disease )
4. Sometimes if paracetamol do not work we may need opiod like tramadol to be used for pain relief .if needed this can be used long term.

5. Physiotherapy to stenthern shouder muscles may aslo help .
6. Topical pain killer cream to be aplied on joints may avoid need of systemic drugs but may not work very well in shouder joints .

7. Surgory ( joint replacement) may be performed in severely advanced disease ( if life is severely impaired ,specially in knee or hip joint involvent ) However shoulder joints would rarley need that as it does not go that advance .

8. You can try chondrotine sulphate and hylaronic acid ( joint support complex, available over the counter) for one month if there was improvement you can use it long time , if no improvement in 1 month then stop it.
I will aslo recommend some vit D supliments and calsiun.

Most of the time shoulder joints osteoarthritis doesnot cause too much problems and most people have a good quality of life.

The course of this disease in unpredictable. some Peopel get painful joints for some times then get pain free for good time and this may happen on and off.Some people are pain free for a very long time .So you should not worry alot .Most people have a normal life with some painkiller needed off and on.
