What Is The Cause And Treatment For Acute UTI?

Question: Hi Doctor ,
My mother who is 62 yrs old has been treated for some kind of infectoion of urine and kidney . She is both Diabetic ad Thyroid . She ha already undergone 10 days of intraevenous antibiotic treatement . She has lost lot of weight and very weak . She is showing on improvement in he urine analysis which says "FULL OF PUS CELLS".

Please advise what should be our nex step.

Thanks for your query,based on the facts that you have posted it appears that your 63 years old mother has Acute UTI.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1)Since how long she has this problem?
2) Was her urine culture and sensitivity test done.?

Acute UTI at this age in females is primarily due to what is called as Post Menopausal Urethral Stenosis where all the Genito Urinary organs undergo changes of atrophy which results in narrowing of urethral passage (Urethra) causing incomplete emptying of bladder during urination resulting into stagnation of urine in bladder which gets infected giving rise to symptoms of UTI.

Please consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis.
1) Routine urine test and urine culture done to find out the organisums causing this infection and antibiotics to which they are sensitive to.
2) Endoscopic Examination Bladder (Cystoscopy).

Please give her broad spectrum antibiotics like Cefexine along with urinary antiseptic like Nitrofurantoin twice daily and urine alkaliser thrice daily.Later on switch on to appropriate antibiotics as per culture report.

Urethral Calibration done while doing Cystoscopy followed by long term catheterization will help to resolve the issue.

Proper control of diabetes with Insulin and oral hypoglycaemic drugs is mandatory .

Thanks Doctor ,

I would like to add some history of mistiy mother's problem .
She has been a diabetic and thyroid pateint for around 15 yrs now .
She has been taking medicines for the same but frankly not very regular also her diabetes is not very controlled ranges 300+ .

Last one year history:
April 2013 :
She had an accident where she broke 3 of her bones and was given treatment for it . Within a month of accident she travelled and was recovering slowly.

July-August 2013.

She had lot of weight loss ( almost bony ) and was fatigued with loss of appetite.
She was many times unable to get up from bed on her own as she had turned so weak .
During these 3 months we could not had her diabetes checked .

Sept -December 2013 .
Condition continued and her diabetes reported high and was taking medication to control it .Again fatigued and loss of weight was high.

January 2014 .
She was very weak and urinated in her clothes and was unable to get up with high fever . She was hospitalized and was given some intravenous to improve the condition for 3-4 days . Ultrasound was done . and intravenous injections were managed for 7 days once daily .
She got slighlty better and got some strength in some time but still was very weak . She never complained of pain while urination
doctor told in layman that all reports are ok except ultrasound according to which some "renal infection" which is nothing to worry about but should be ok with medication.

March 2014.
As her weight did not improved we moved to another doctor who based on urine reports indciated pus cessl 80-100 and hecne provide 10 days of intravenous injections . After that the pus cell count did not reduce .


April 2014.

We move to another doctor who said it is high diabetes which needs to be controlled to remove the infection problem . He did not provide any intravenous injections but gave medicatiion ( on insulin ). With the latest diabetes reports it is improved diabetes quite a lot but still not fully controlled ~150.

However her urine culture was done 2 days back which shows 80-100 pus cells and improvement in diabeters.
So the doctor is concerned and saying need to talk to gynaecologist and go for ultrasound .

She has somewhat improvement on physical sie but still very weak .

Please advise us .

Thanks for follow up and highlighting details over a period of last year.

One thing is sure that she had severe UTI due to uncontrolled diabetes.

In my opinion, she should get her Renal function test done to rule out Diabetic Nephropathy which is very common in in a patient with uncontrolled diabetes. This can be a cause for her general debility.

If urine culture is done start appropriate antibiotics as per culture report .

With proper control of diabetes and urinary infection she is bound to recover soon.

Will take your options.
But i have few questions for you

1. There has been full courses of intravenous injection more than once. Inspite of it she has no improvement on infection reduction as report has 'full of pus cell'. Is there for some reason ?

2. Although since XXXXXXX 2014 she has gained some energy but she has not grown weight and has lost lot of weigt. (43 kilo for her height of 5'5"). She is very bony and no weight gain and fatigued.

3. Although her diabets is around 150 now do we expect her condition to improve would take months from now ?

4. Finally, why did the physician we visited last asked for gynaecologist ? Her uterus was removed 20 yrs back.

Thanks for writing again.
Was urine and culture and sensitivity done prior to start intravenous antibiotics ?Antibiotics given by intravenous route does not necessarily mean to give results unless the bacteria causing infection are sensitive to particular antibiotics.One may not see improvement if the bacteria are resistant to the said antibiotics.

2)Please get her Renal Function Tests done to rule out Diabetic Nephropathy.

3)There are chances of improvement provided her diabetes is controlled properly.

4) I can not comment on some ones opinion.
