Brief Answer:
oral and topical antifungals, antifungal shampoo

Detailed Answer:
Hello,
Thank you for posting on HCM.

From keen review of your history and description, it seems you might be having seborrheic dermatitis. Its a kind of chronic inflammatory skin condition involving sebaceous glands i.e oil producing glands of the body and it is also dictated by certain fungal like Malessizia.
Combination of inflammation, excess oil or sebum production and fungal overgrowth produces characteristic flaky plaques on scalp, behind ears, and around nose. These lesions can emanate some foul odor.

Generally in my patients of seborrheic dermatitis, I prescribe a short course of oral antifungal drugs like ketoconazole or fluconazole.
For daily application, I recommend a clotrimazole and betamethasone based lotion and cream over affected areas twice a day for 6-8 weeks.
To cleanse the scalp of foul smelling plaques and flakes, use ketoconazle, ZPTO and salicyclic acid based shampoo twice or thrice a week.
Use ketoconazole soap to rinse underarms and groin area.
Avoid excessive use of hair oil.

I am sure within few weeks of treatment, you will be pretty fine
Let know about your situation after 3 weeks.
Please let me know if I can be of any more assistance to you.
Thank you
Take care

Brief Answer:
Oral and topical antifungals, topical clobetasol

Detailed Answer:
Hello XXXX

Thank you for the photographs.

From observating your pics it looks pretty much like seborrheic dermatitis. But I would like to keep a differential diagnosis of scalp psoriasis.

Anyways the treatment remains almost the same.

Take oral ketoconazole tablet 200mg (NIZRAL) once daily after lunch for 15 days.
Apply a lotion containing clobetasol and salicyclic acid ( CLOP-S Nano lotion) and a lotion containing Sertaconazole (ONABET lotion). Mix both the lotion in 1:1 ratio and apply twice a day over affected scalp.

Use ketokonazole and salicylic acid based shampoo thrice a week.( SALISIA- KT)

To reduce the foul odour I would suggest one simple treatment. Get a packet potassium permanganate crystals. Take just 2-3 tiny crystals and add to half bucket full of lukewarm water, stir gently to dissolve crystals and you would get pale pink color of solution.(Match color with the color of your nail bed as a guide!). Use this solution on daily basis to rinse your scalp.

Please discuss all these treatment options with your dermatologist and take the prescriptions accordingly.

I am sure just one week of this treatment and you would be doing fine.

Hope the information was satisfactory.
Thank you
