What Is The Cause And Treatment For Recurring Fever And Pain In Neck?

Question: Patient (my mother) has been suffering from fever repeatedly occurring for the last more than 26 days. Had felt little stretch & pain while touching from out side of the neck and had been getting little Bulgum. AND she had local MBBS doctor's prescription a normal anti-biotic and paracitamol.

After that went to ENT specialist, he told there is nothing as such in throat. When its been 9-10 days. we went for complete blood & urine tests CBC / Widal / MP etc.... everything was normal BUT TLC was arround 12000 and ESR 78. On 15th day she got TLC 16500.

Blood & urine culture is awaited. ALSO saliva / bulgum culture is awaited.
Patient has given her blood sample for all types of thyroid test. (REPORTS enclosed.)

It tells something wrong in the both sides of neck, which doctor told as swelling in thyroid gland due to some growth arround it. Equally thyroid reports are telling something corelated.

NOW, WE HAVE GOT TECHNITIUM UPTAKE SCAN DONE, please find enclosed.
1. all blood test reports (except culture)
2. all urine test reports (except culture)
3. Abdomen ultra sound
4. Neck ultra sound.
5. Thyriod Test reports.
6. Sputum culture is also awaited.
7. Technitium Upstake Scan
8. Overall summary 2 pages.

KINDLY GO THROUGH THE SAME and respond with your expertise opinion. Blood, Urine, Sputum all three culture reports are awaited. I shall update as soon as received.

Dear XXXX, hi! your mother has been suffering from acute thyroiditis. she doe not need antibiotics, she only needs lot of fluids, rest, and indomethacin 75 mg. she also has low vitamin B12, she will also need b12 supplemments alomng with calcium and vitamin D. 26 days have already passed , this phase usually lasts for 6 weeks. so bear with her for another 4 weeks probably. repeat TSH FT4 after a month and let me know. continue calcium 1gm, b12 1500 mcg, evry day. and vitamin D 60000IU once a week for 12 weeks. regards!

We have got Sputum Culture, Blood Culture reports as well. I shall upload
now.

1. We are not made clear by our local doctors about the reason/cause of thyrioditis.
Please throw some light on the causes/reason of this disease.

OR if there is some other common health issue, which, if remained ignored, may affect Thyriod.

OR if there is any medicine / health supplement which might have affected this.

She use to take combiflame for headache or any other muscle pain/stretch.
and also took calcium course some time back.


2. Kindly also detail precautions in future keeping in view Thyrioditis.

3. Kindly also tell us Diet restrictions if any, keeping in view Thyrioditis.

Dear ma'm, i was out of my office so i could not answer your follow up query timely.
1. thyroiditis can be infective or autoimmune. it can be viral , bacterial.
2. thyroiditis may leave you hypothyroid for life. so there is nothing like repeat attack. yes if she becomes normal, she might have repeat episode, but very less likely. she can check her TPO/ TGA antibodies to know the auto immune profile.
3. no food or life style precaution is directly linked to thyroiditis but in general healthy lifestyle to keep your immune status high is what i can recommend. regards!
