What Is The Difference Between Prostatitis And Mild Prostatomegaly
Question: My name is XXXX , 27 year old male !
I want to know is there any difference between prostatitis and mild prostatomegaly or they are one and the same thing because earlier ultrasound report of mine examined prostatitis where in current report examined mild prostatomegaly?
My finding of current ultrasound report is as follows -
Both kidneys are normal in size and echotexture with well maintained corticomedullary differentiation...... R.K.: 10.1 x 3.2 cm and L.K.: 10.1 x 3.9 cm
No evidence of hydronephrosis or calculi seen.
No evidence of retroperitoneal or pelvic lymphadenopathy seen
No free fluid seen in the peritoneal cavity
Urinary bladder is normal in contour , capacity ,wall thickness and contents .
Prostate is enlarged measures 3.1 x 4.1x 3.6 cm in size , approx 25 GMs by weight and normal in echotexture. No focal lesion seen .
There is no significant post void residual urine volume .
Impression : findings are s/o mild prostatomegaly.
What should be the normal prostate size of mine as against 25 gms?
I am currently taking medicines named flotrol 10 mg once a day and oxalo twice a day from past one month.
I feel slight pain in penis whether I pass urine or not and even when I masturbate I feel slight pain . What is the cause of this pain and How to regain my sexual life again ?
I have heard about prostate massage and eps needs to be examined and 3D prostate treatment will this help in curing my problem and if yes then what should be duration and how long a person should get prostate massage done?
Benefits of prostate massage is temporary or permanent ?
Does regular masturbation helps in getting infection out?
I am afraid to ask does this problem proves hindrance in the birth of a child?
There is a terminal dribbling .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have any urinary symptoms like burning urination, frequency, nocturia etc?
2) Do you have painful ejaculation?
3) Do you have pain in perineal region?
Your ultrasound scanning report does not reveal any enlargement of prostate. Size of a prostate in normal individual is 20-22 gms. Marginal increase in size can be due to infection(Prostatitis).
Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to find out the cause of your symptoms
1) Routine urine test, urine culture and semen culture done to find out the organisms causing this infection and antibiotics to which they are sensitive to so that you can take appropriate antibiotic as per culture report to combat infection.
Please take broad spectrum antibiotics like Cefexime along with urinary antiseptic like Nitrofurantoin twice daily. Later on switch on to appropriate antibiotics as per culture report. Continue with Flotrol.
Get the prescription of medicines from your family Physician.
Ensure to drink more water. To keep your urine dilute.
As regards your other queries I would like to answer them point to point,
1) Prostatitis is infection of Prostate mostly due to Bacteria. Where as prostatomegaly is enlargement of prostate, it could be either due to infection or due to ageing ( Which is unlikely in your case )
2) Normally we advise prostatic massage when there are no satisfactory results with long term course of antibiotics.( 4-6) weeks.
3) Prostate massage is advised only in refracted cases or Chronic Prostatitis not responding to medications.
4) It will not have any adverse effect on your sexual potency or capacity to impregnate your wife provided you take the treatment religiously.
5) This problem can be cured completely and you will have normal life.

Thanks for answering my query , where in I would like to add some points such as -
My symptoms are like I have a terminal dribbling at the end of passing an urine means just at the end time of passing urine flow slows down a bit, why it is so ?
Secondly I have slight pain in my penis whether I pass urine or not and even when I masturbate so there is nothing like when I am masturbating pain arises, therefore it is a slight pain present all the time , due to which I don't enjoy to masturbate as I used to at one time which is really causing a stress on my mind whether I will be able to regain my sexual life again n enjoy it?
I am really concerned about prostate enlargement as it is stated in my ultrasound report as 25 gms!
I don't have any other symptom other than this .
I have got my routine urine and culture reports examined which are normal .
My doctor has underwent digital rectal examination to feel the prostate inflammatory but has not undergone the testing of EPS to identify the infection rather prescribed an antibiotics without examining it to me such as bactrim , zinase and piltop for 1 month then changed it to flotrol 10 mg and oxalo which I am taking from past 1 month .
I really don't have any idea whether i am getting right antibiotics or not
Although slight pain in my penis as mentioned above has reduced significantly but not yet completely gone which is a cause of my worry and hindrance for me to have a healthy sexual life .
He has not prescribed me for semen culture as of now .
I have read a lot on prostatitis where in there are two types of it acute and chronic I don't know from which one I am passing through and how much long it will take to get it cured
I have heard about prostate massage n drainage is necessary to perform every two days and examine culture of Eps both left and right side of prostate gland every two days in order to give antibiotics access to the places infection has been clogged where in normal cases antibiotics can't reach alone and is very helpful to shrink prostate enlargement and to gain sexual life again as it helps in disappearing the pain caused and other symptoms by secreating the infected fluids from urethra but really don't know whether it is required in my stage , how to perform it n all?
3D treatments of China to treat prostatitis is to inject antibiotics directly to the infected area to kill pathogens from pelvic area ......is this kind of treatment present in XXXXXXX and is effective?

Since you have been taking antibiotics for almost 2 months with not satisfactory results it could bedue to one of the following two reasons.
1) Organisms causing infection are not sensitive to antibiotics that you have taken.
2) It may be a abacterial prostatitis
Since antibiotics were prescribed without the knowledge of organisms and their sensitivity pattern I would suggest you to get swab test and culture of a urethral discharge obtained after prostate massage .You will need help of your family physician or urologist to get prostate massage done properly in order to get prostatic fluid for test.
We do advise prostate massage as a part of treatment and it does help to resolve the symptoms .However it has to done either one of your close relative after learning howto do it from qualified Dr.else you can get it done by a qualified nursing staff.
We do not advocate giving injection in prostate .
