Dear Doctor,
I am 73 year old man.I had prediabetes 4 years back. I had good control over diabetes all these years. But since an year i have been observing that my fasting level is hovering between 115 to 125. My post lunch levels are always within the range below 140.i have been taking 250 mg glycomet after breakfast and 500 mg after dinner.The values are as under.
levels as on May 2015 Nove 2015 may 2016
Fasting 105 120 122
HBA1 c 6.6 6-7 6.8
My question is whether i have to take 500 mg instead of 250 mg as at present after breakfast also. i.e 1000 mg per day.?
Since i have been taking the glycomet always after breakfast and after dinner, If i change it to take before food does this have a better effect over diabetes?
Also please advise me how to control tryglycerides .At present it is 196. My cholestrol level is 144 . Last year at the same time it is 135 and 123 respectively.I am taking Ecosprin AV 75 one a day.Do i have to take statin separately in addition to ecosprin av 75?.Kindly advise.

Hi,
Welcoem to HCM.

I have read your query and understand the concern.

1) Fasting blood sugar between 115 to 125 is considered as a normal and post prandial blood suagr below 150 is considered as a clinically normal.

Your HBA1c values is also not high or I would say above 7% is considered as a clinically significant.

So,I do not think there is any need to change the dose of glycomet in your case.You continue taking 250 mg glycomet after breakfast and 500 mg after dinner as directed by your doctor.

Also,there is not a much difference if you take glycomet (anti-diabetic drug) before or after meal.It usually do not affect drug efficacy or drug effectiveness whether you take it before or after meal.

However,I suggest my patients to take it just before meal means just 5 minutes before starting meal.

2) 196 triglyceride value is a borderline high (but not too high) and it can be controlled well with dietary measures,so there is no need to start statins (like atorvastatin) separately or in combination with ecosprion.

You can follow below mentioed dietary precautions.

Avoid direct sugars and avoid fried snacks, bakery items.

Take ample of vegetable salads- at least 1 bowlful cut raw vegetables like cucumber, tomato, carrot and vegetables which you can eat raw.

Snacks should contain healthy nuts like almonds or walnuts and a few dry fruits but no fruit juices.

Avoid soft drinks or hard drinks and smoking cessation if you do so.

Skimmed milk and milk items made up of skimmed milk are good to check on cholesterol and triglyceride.

Egg white is fine but avoid egg yellow means yolk.
