Question: What is the maximum safe amount of Lomotil that can be taken daily to control diarrhea caused by IV and oral antibiotics for cellulitis infection subsequent to temporary cessation of Revlimid for multiple myeloma.

I am sorry to hear that you are bothered by diarrhea associated with antibiotics. The reason you get diarrhea is that the antibiotics kill the bacteria flora of the GI tract. I always recommend when taking antibiotics that you also take a probiotic. This will help minimize the diarrhea. If you do get diarrhea then lomotil is an option to reduce the number of bowel movements. The usual dose of lomotil is 1-2 tablets every loose BM with a XXXXXXX dose of 8 tablets per day. You must only take the lomotil for 48 hours only. If the diarrhea continues then you may have developed a super infection called Clostridium dificile. This is a potential dangerous infection and needs addressed early.

In Summary
1. Maximum dose 8 tablets daily or 40ml/day
2. Use a probiotic to prevent or reduce diarrhea
3. If diarrhea persists more then 48 hours be concerned with Clostridium dificile infection

Thank you. I have been tested for c. difficile at least twice, and have tested negative. The Lomotil was prescribed after that second test for c. difficile with no limitation on the duration to take it.

I will try a probiotic on a more consistent basis. Do you recommend Align? My daughter uses a probiotic called Five Lac from Global Health Trax and thinks it is excellent.

How frequently should the probiotic be taken?

Any of the probiotics will work. Align is just as good as another. There has been no studies to show which is superior to another because there are no head to head studies. For align to work properly I recommend in taking the probiotic 3-4 times a day. You really can't overdose on the probiotic.

Good luck and I am very happy to hear that you do not have the C. difficile infection as it can be very difficult to cure.
