What Is The Dosage Of Synthroid For Hashimoto's Disease?

Question: Hi there,
I have Hashimotos for the last 15 years and take Synthroid. I never had problems with symptoms until recently. I finally am feeling a little better after learning that I was on way too much Synthroid and then way too little Synthroid. I still don't feel GREAT, but I feel a lot better than I was. My current dose of Synthroid is .112 mcg Monday through Friday and .100mcg on Saturday and Sunday. This dose seems to be doing well with me. I do have symptoms some days, but I have more good days than bad. About 3 months ago, my new doctor tested my reverse t3 and said that it was borderline high at 24.5, so she added Cytomel to my regimen and I only took it for 2 days and could not take it. It sent my heart racing and pounding. My doctor told me to stop taking it because my heart rate was too high. I have been doing ok and had my labs drawn this week. My tsh is 0.5, my Free t4 is 1.79 and my reverse t3 is 25.5. Because I have been feeling a little hyperthyroid, she told me to take .112mcg 4 days a week and in between, take .100mcg 3 days per week. Can having a borderline high Reverse T3 hurt me? Can I just live with it that way forever? I don't want any long term damage to my body. Thank you!

Good day.
Noted your concern. The active Thyroid hormone is Free T3. Reverse T3 is an inactive from of T3. However routine screening of reverse T3 is not recommended. A doctor should treat the patient rather than than lab reports. If you felt great on the previous regimen of synthroid, you should continue the same. Reverse T3 increases when there is fever or stress.

As long as Free T4 and TSH are normal, no need to change the synthroid dose. Use of cytomel (T3) is not recommended in routine clinical use except in special conditions like myxedema coma.

I feel it is reasonable for you to use synthroid 112 ×4 days and 100 X 3 days. Please recheck TSH after 6 weeks on this new regimen.

So I should not worry about the reverse t3 numbers, correct? These are not important? I just want to make sure. I do have mild anxiety and have a toxic reaction to SSRIs and SNRIs. I cannot take them, so I have to try to handle my anxiety in other ways. I tend to only get anxiety when my thyroid levels are off and I am overmedicated since I wake up in the morning with a racing heart and pressure in my throat. When my thyroid levels are right, I have no anxiety symptoms. I am going to use synthroid 112 4 days and 100 for 3 days. I just wanted to make sure that having an abundance of reverse t3 can't hurt me physically.

If your Free T3 and Free T4 and normal and if TSH is good as well, please do not worry about reverse T3.

Please recheck TSH after 6 weeks of starting the new synthroid regimen.

May I advice you regarding breathing exercise/Yoga and meditation to handle the stress/anxiety. They are extremely helpful if you practice regularly

Thank you so much! I do feel better when I exercise. I usually do bike riding 4-5 miles per day. When I miss those days, I feel bad. Thank you again! I will try not to worry about it.
