Question: Hi Doctor, Last year i went for prenatal advice to my gynec, she then suggested me for a blood test, HCG level came high, so she suggested to go for another blood test in fasting. On 20/12/2014 the results came as T3 - 1.05ng/ml, T4 - 10.9ug/dl and TSH as 2.44 uIU/ml. So she said everything is fine. Now i am pregnant of 12-13 weeks. My last periods was on 16th of XXXXXXX 2015, in the first blood test after conceiving the TSH level was 6.08uIU/ml on 1/8/2015. Gynec advised to go for FT3 and FT4 test and visit a general physician, on 5/8/2015 results came as FT3 - 2.52 pg/ml and FT4 - 1.11 ng/dl. General physician suggested to take half a tablet of thyronorm 12.5 everyday morning before breakfast. I have read that TSH levels are very important for the baby in the first trimester. Will this be a problem for my baby?

Dear XXXXXXX Hi! I am dr XXXXXXX endocrinologist. Yes you have rightly heard maintaining a normal thyroid balance is very essential all through life and specially so during pregnancy and lactation.
normal balance we target during pregnancy is
1. TSH around 1uIU/ML
2. Free T4 >1.4 ng/dl.
now it is easy for you to interpret that you had higher TSH than the target and lower FT4 .
so you definitely needed the thyroxine replacement which your physician has already given you.
but the dose which you have been advised is very less. you need much more than that. your body weight will determine your initial dose. say for eg if my patients are 60 kg I prescribe them 75 mcg /day.
you must take the tablet empty stomach early morning and observe another 30 minutes fasting after the tab.
another point to remember is that you need close monitoring through out the pregnancy. so please recheck your TSH /FT4 every 4-6 weeks and also recheck with your doctor to change the dosage.
