Question: Hello. What in your opinion is thd main determinant for successful ivf pregnancy/live birth in women over40: is it age, fsh, amh, lh or any other factors?

Main determinant for success in IVF is age. Age determines quality and quantity of eggs in a lady especially 40 yrs and above.

FSH and AMH are just guide to ovarian reserve.

Hi,

Fibroids seen on picture are not clear and difficult to comment.

If the fibroid size is large or encroaching on cavity, it is better to operate and get them removed. These fibroids if are on endometrium, they affect implantation as well as pregnancy.

The surgery is called myomectomy. It can be done by laparoscopy. If you are not willing for myomectomy get hysteroscopy done to diagnose if fibroids are affecting endometrial cavity.

The myomas are described as fundal 33.7X31mm; interstitial anterior 27.2x27.2mm, and interstitial posterior 31.1x31.2mm.
The report attached to the ultrasound states: "Multiple uteri fybromyoma. Uterus of larger volume than the norm, because of the presence of several fybrous nodules (photos attached)."
Uterus L-9.38cm, W-6.72cm, H-5.93cm, Volume-195.7cm3, Endo.Thickness-6.74cm.
From the term "interstitial" these do not appear to be encroaching on cavity or endometrium. Is that correct?

Hi,

From reports it does not seem to be encroaching the cavity. Especially interstitial.

But fibroids are large and multiple. Hence it is better to undergo hysteroscopy for assessment of uterine cavity before proceeding to IVF
