Question : What is the process to get off PPIs and H2 Blockers ? It has been 2 weeks since PPI stopped. Should one try Pepsigard, or slowly reduce dose /frequency of Zantac ? Or do both ?



My wife had been on Nexpro for a month ( after v mild H Pylori treatment for 7 days). When she stopped, she had acidity symptoms as expected. She took H2 Blockers Zantac twice a day for a few days and then was able to get it to once a day. However, in attempting to stop it completely she faced acidity and so Zontac dose is being taken in an irregular, 24-30 hour gap.

Have seen your details,
Very likely your wife is suffering from Gastritis or acid peptic disorder.
It's difficult to cure gastritis however in long terms patient symptoms can be controlled.
The best way is to start a PPI for 3 months initially.
Like NEXIUM 40 mg twice daily followed by NEXIUM 20 mg twice daily followed by NEXIUM 20mg in morning and Rantac300 mg in evening followed by Rantac300 mg twice daily followed by Rantac 150 mg once daily.
After taking Rantac 150 mg once if you feel better after 3 months then try to stop it however if symptoms bounce back again start taking 150mg Rantac once daily. Weaning off is very difficult in gastritis patient. It's a slow process and patience is required.

Endoscopy had stated some antral gastritis and v mild H Pylori, but the doctor had not considered it to be significant, and told us to just stop the Nexpro after 1 month of H pylori treatment, not slowly, H2 blocker Zantac was given for SOS use. ( I personally never thought this would be easy, and a slow tapering like you stated would be needed).

Do you think Pepsigard o will help in the process when combined with Rantac (Rantidine) or do you think Rantac 150 mg twice is enough ?

Hi again,
Pepsigaurd o does not play a significant role in her case as she has got gastritis and not ulcers. In place of it you can add mucaine gel three times a day which can be much more helpful(10ml).Sucralfate is used to heal ulcers.(pepsigard o). Don't be in any haste. Gastritis is very difficult to cure and easy to control. Just taper things slowly and only after consultation.
