What Is The Risk Associated With Tapping The Water From The Scrotal Sac For A Patient With Liver Cirrhosis And Hepatitis-B?

Question: hi, this is about someone of age 63, male. He is a cirroshis of lever patient - he got it 7 years of ago from jaundice followed by hepatitis-b (he's a teetotaller). he developed ascitis 6/7 months back and got water accumulated in abdomen, legs and scrotal sac. after continuous treatment and removing water from abdomen through tapping few times, now apparently no water accumulation is happening. the legs and abdomen have got back to their original shape, however, the scrotal sac doesn't seem to be showing any improvement in terms of releasing accumulated water. it's grown quite big and is a huge inconvenience for moving around. my question is: what is the risk associated with tapping the water from scrotal sac like it was done from abdomen? is it done anywhere in Hyderabad? thank you.

thanks very much doctor for your quick response! he has no issue related to bp, diabetes, urination etc. he undergoes all routine tests regularly and never been diagnosed with anything related to cardiac stuff. its only the liver it seems. most recent bp was monitored 2 days ago and it was 120/70. attached are the recent reports. kindly suggest if anything can be done to expedite removal of water from scrotal sac or how long it may generally take. the volume there looks to be around 1L and it gives a huge bulge.

Hello Sir,
Thank you for going through the reports and maintaining interest in my question. Kindly refer to the reports (9,10,11) I uploaded recently. If somehow the water can be removed on a one-time basis, then that would be a great relief for him. We are hoping it'll not be accumulated again since the secretion seems to have stopped. Thank you!

Thanks for remaining reports.

* The ultrasound of groin and scrotum is showing gross hydrocele on right side
with hernia defect of 31x27 mm size ( which means there is intestine loop going
into scrotum ).

* In such condition, only tapping of fluid from scrotum is not advisable and
feasible at all.

* My recommendation to cure this issue is to get him operated for
Right sided orchidectomy with inguinal hernia mesh repair for permanent
solution provided he is fit to undergo surgery under spinal anesthesia.
