Question: Looking for a doctor in the Washington D.C. metropolitan area capable of treating akathisia believed to have been induced from high dosages of prochlorperazine ( Compazine) prescribed to control nausea associated with chem-therapy treatment? Do you have any recommendations?

Akathisia is a Akathisa is one of the most common and distressing neuroleptic-induced extrapyramidal side effects from first and second generation antipsychotics. Prochlorperazine is an antipsychotic which works like a prokinetic in vomiting and vertigo etc. As you have a malignancy of Colorectal CA and you are on chemo. Usually some thing more than Prochlorperazine is needed like Ondansetron and some other options like dexamethasone, cannabinoid, Dopamine receptor antagonist,Benzodiazepines and Aprepitant. Mean while Eat small, frequent meals or snacks. Drink plenty of water and non caffeinated liquids to avoid dehydration. Avoid greasy or spicy foods. Eat dry foods such as crackers, toast, or dry cereals. Eat soft, bland foods that are easy to digest. Have a strict monitoring of electrolytes and hydration status to prevent troubles. You need a clinical dietician for your electrolytes and hydration status and a psychologist for anticipatory Nausea and vomiting and prochlorperazine replacement and an oncologist for chemo induced vomiting. Her dosage of compazine was 10MG every six hours over a two month period of time ending last Wednesday (four days ago) and she is still experincing akathisia symptoms - is there any expected approximate time period over which the symptoms should subisde enirely?

Brief Answer: Mirtazapine & Benztropine Detailed Answer: Thank you for getting back to me! Akathisia is an extra pyramidal side effect of Antipsychotics the patient took for vomiting post chemo. Now the trouble of persistence of akathisia despite stopping the drug needs a bit work up to see how damaged the functions of extrapyramidal symptoms are. Benzotropines and other anticholinergics and drugs specially like Mirtazapine has shown promising effects of Mirtazapine. Instead of taking compazine 10 mg for every 6 hours she could have been advised with mornin 15 mg or it as that would have spared the odds for Akathisia. Its still manageable. get a psychiatrist and let them decide how to help it. Medicines i mentioned will be used to relieve the akathisia symptoms. Good Luck
