Posted on Tue, 31 Dec 2013
Question: Hello Doctor, I am facing problem with Pimples. When the pimple goes off, it leaves a dark spot on my face which takes very long to fade away. I am facing this problem from last 2 years or so. Only 3 months back I consulted a doctor who recommended me to use Benzoyl Peroxide along with Azithral tablet. It didn't helped much. After consulting another doctor only few days back he prescribed supatret C along with minoz 50. Should I use that? This is for your information that I see pimples on my face 1 or 2 days after I masturbate. When I stop doing that I don't see any pimples coming up. So I have stopped enjoying myself in that way but still there are many dark spots left out. I am also using home made therapy such as: applying honey, besan+haldi+milk. Should I continue doing so? Will that be helpful in long run. Please help me. I am getting engaged in another 45 days and I want to be spotless free. Many thanks for your valuable suggestions.

Brief Answer: No home remedies please Detailed Answer: Hi XXXXXXX Welcome and thanks for posting your query to Healthcare magic. I can understand your concern for the pimples and dark spots. Before I proceed to your issue let me explain you elaborately regarding pimples- Acne or pimples in most of teenage people starts appearing at the age of 13-14 years and then mostly vanishes by the age of 25-30 years. In some persons it can be prolonged till the age of 30 years also. The main cause of pimple formation is basically due to hormones (chemicals which are influencing one or other body functions) in addition to the influence of certain other factors causing its aggravation. The hormone plays the most important role in high oil secretion and increase the size of oil glands. As the pores or ducts of the glands are blocked so it leads to formation of pimples. There are certain other factors such as application of oily or creamy products, over exposure to sunlight, heat and humidity or due to pinching, popping and squeezing of the pimples also leads to formation of pimples. Once these are healing they leave the most problematic thing which are either marks or scars. Therefore any of the above factors along with hormones can cause appearance of pimples before the age of 25-30 years. After the age of 25 years there is less upsurge in the hormones and these gets stabilized and therefore less appearance of pimples after 25 years. So probably you got some idea regarding your acne. There is no correlation of maturation with acne. Even then if you feel that there is formation of pimples after maturation then you can go for it accordingly. There are certain best of suggestions which you can follow: 1. Do not pinch, pop or squeeze any area on face. 2. Do not apply any creamy or oily stuff or any other cosmetics over face. 3. Try to avoid any home remedies (applying honey, besan+haldi+milk.)suggested by your friends or family members or otherwise self implicated therapies also. Most of he times any of the home remedies blocks more of the pores and results in more aggarvation of pimples. 4. Do not take steam or steam bath or massage on your face as these also aggravates the pimples. 5. Try avoiding situations of direct continuous exposure to sunlight or heat and humidity for long hours. 6. Kindly do not use any antibiotic as self medication. 7. Do not apply any hair oil on scalp or apply in very less amount. Wash your scalp daily with some good shampoo and let there be no dandruff in your scalp ( because dandruff itself causes blockage of pores of oil glands and then pimples). For the part of best treatment kindly give me brief idea regarding the status of spots and pimples so that I can suggest you further: - status of your pimples - how often in a week/month you get big pimples. - Are there any deep scars and/ or very dark pigmentation on face. It would be better if you can upload a close picture of face. For that you have a feature to upload images by yourself at the right side of the query page, please utilize that so that I can answer your further queries. Or else, you can send as an attachment to YYYY@YYYY with the subject as "Attn: Dr. Sanjay Kumar Kanodia). Hope to hear soon from your side. Regards. Dr Sanjay Kumar Kanodia MD (Dermatology)

Brief Answer: KInldy follow the suggestions below Detailed Answer: Hello, Welcome again and thanks for posting your further details and pictures so swiftly. After analyzing the pictures properly I can make out that you have basically the spots rather than pimples which are making the things on negative side. I can understand your concern for the dark pigmentation problem. It is really distressing for anyone to have anything unusual on face. The present mode of treatment given to you (supatret C along with minoz 50) is basically meant for the acne part. As you do not have much of pimples so my basic concern is towards spots. This type of Pigmentation is a common phenomenon because of more activity of the color producing cells know as "melanocytes". These melanocytes are active because of multiple causes but one of the most important cause is because of exposure to Ultraviolet radiations and repeatedly touching or pinching or popping the pimples. Kindly follow the suggestions below: - The most important measure is to stay away from direct sun light by any means. Do not expose yourself to sort of sunlight which is direct or indirect. Even during this cloudy weather when direct sun is not present then also you should give ample sun protection. - Do not pinch, pop or squeeze any area on face. I request you to not een touch your face. During washing your face you be gentle. - You can apply a good sunscreen (Ultraguard 40) over entire face in a very thin layer for 3-4 times. Apply it in such a thin layer that it should not be visible by naked eye. Do not rub it. You say that you are already applying sunscreen. The important fact is that it is to be applied half an hour before going out. The sunscreen are effective for 3 hours only, so if the direct or indirect exposure is present then reapply after eery 3 hours. - You can take any antioxidant and multivitamin along with the above especially those containing high amount of Vitamin A and vitamin C in it (like Antoxid HC or Perfectil). - For the lightening creams/ pigment reducing cream I general prescribe Kojic acid cream (like Clearz ultra ) and Azelaic acid cream ( Aziderm 10 %) at night time only over the affected area. These can be applied one after the other on alternate day basis at night time only. Apply it in such a thin layer that it should not be visible by naked eye. Do not try to rub it. These creams are to be used under dermatological guidance only. Azelaic acid cream will give you an additional advantage of reducing the pimples too. SO you can use it over active pimples also. - Have plenty of fruits containing vitamin C and A. As you have your engagement after 45 days so till that time you will get good benefit. For the best benefit you have to wait for atleast 3-4 months to see the results, so have patience. Do not go for any massage or any parlor activity at any beauty shop or parlors as it can lead to activation of pimples. I hope these informations will help you. If you have further queries I will be happy to help. All the best wishes for your forth coming engagement. "Wish you good glowing skin" Regards, Dr Sanjay Kumar Kanodia MD (Dermatology)

Thanks Dr. Is it okay to go ahead with supatret C along with minoz 50 OR should i apply Kojic acid cream (like Clearz ultra ) and Azelaic acid cream ( Aziderm 10 %). Do I need to apply both the creams or any one of them ( Kojic acid cream (like Clearz ultra ) AND/OR Azelaic acid cream ( Aziderm 10 %)). Please suggest where should I start with. Thanks again!!!

Brief Answer: Kindly follow the details Detailed Answer: Hi XXXXXXX Thanks for further follow up. If I were your treating doctor I would have suggested to use both Kojic acid and azelaic acid creams on alternate night basis ( Kojic cream at one night and azelaic at the other). As you do not have much of the pimples so I don't find helpful in using supatret C along with minoz 50. Also to tell you importantly that Minoz contains Minocycline which is basically antibiotic though very effective for pimples but can cause the effect of hyper-pigmentation. So in my patients I do not use Minocycline with pigmentation or spot tendency. I hope you must have all your answers. If you have further queries I will be happy to help or if you do not then an close the query and rate the answer. "Wish you good health" Regards, Dr Sanjay Kumar Kanodia MD (Dermatology)
