Brief Answer:
An infected acrochordon

Detailed Answer:
Hello. Thank you for asking.
I am Dr (Dermatologist and Venereologist). I have noted your concern and I have viewed the image.

Seems like an acrochordon / a skin tag which has probably got infected and that's why it is paining. I suggest you take an oral antibiotic e.g either Amoxicillin or Cephalexin for a week.

In addition, I suggest you take an oral anti-inflammatory e.g. Ibuprofen or Diclofenac for symptomatic relief from pain. Once the infection resolves this skin tag can be snipped off from the base under local anesthesia.

Regards
