Question: What kind of spider or bug bite leaves a nickel sized black welt? We are in Maine, Arizona, Wyoming each year. Have had lyme disease, but am told ticks leave a red welt.

Unknown insect Bite -with Black Welt/ under investigation
I studied it in all the details.
-Your main concerns are understood and are as follows-
While you were on your visit to Maine,Arizona,Wyoming are,
You had a unknown bite, which caused following problems-

a-The nickel size black Welt
b-with current back headaches,
c- right ear area numbing
d-with tendonitis at right wrist
e-Weird popping movement in your upper right leg,which is not near right knee joint-indicating the site of your insect bite.
d-with a history of lyme disease due to ticks bite

I need more info if you can update ASAP-
a-When did the bite occur -what time and which date
b-when you had tick bite last, what was the area you were visiting- was it the same or different than for this bite?
c-Plz update the photograph of the bite site with the black welt you have

In the meanwhile I would suggest following treatment to be taken under guidance from your ER doctor.
a-Continue same drugs as given by your doctor
b-Antibiotics need to be taken to avoid infection at black welt at the bite site.
c-Diclofenac sodium gel locally
d-anti-histamin drugs orally
e-Inj-B1/B6/B-12 once a day

I am horrible for seeking treatments, so I had been bitten a year and a half ago, on my left side torso. The reason I finally went to a urgent care was my left foot is puffy, and that is why they gave me antibiotics. Also, took the antihistamin pills. Lyme situation was in 2007, again I waited too long, infectious disease dr. put me on rocephrin intravenously for 20 days. Went to him for this, but he doesn't like that I don't have a primary care doctor. We travel internationally twice a year as well. Is treating spider bites really so different from ticks, scorpions, black widows?

Dear Good Morning.Thanks for the updated info on your query.

After studying and reviewing the details of your query-I suggest following treatment plan to help your health problem-
My impression of your problem is -Black welt on left foot due to infected unnoticed trauma
a-There is no relation of the Black welt with puffy left foot you have, to the old bite which you had one and half year back.So don't worry about unknown bite causing insects as queried by you and better forget it.
b-By your age and mostly due to neglected trauma to your left foot which later got infected,you have all these current problems of -small headache/numb ear- and are all due to the infection in foot currently,which your doctor has rightly treated.
c-For The right wrist tendinitis -I would advise you to revert back with a fresh direct query to Me at HCM to work up on its cause and treatment,as it could be related with ? lymes disease you had.
d-I would suggest you to take-
i-Tab-Anti-inflammatory drugs-like Voltaren-(diclofenac sodium)
ii-Rest and Elevation of the left foot-
iii-Continue Antibiotic treatment-like Ofloxacin with ornidazole- with your doctor
iv-To attend to surgeon for the would care of the left foot if need be.
In conclusion-
a-there is no relation of the OLD unknown bite(1.5 yrs back) to the black welt of the left foo which you have now.
b-Continue treatment with your doctor for the left foot infection ,which is due to the unnoticed,neglected trauma being complicated by infection.
