1. Bronchitis asthma
2. Thalassemia Minor
3. Severe Iron deficiency anemia
4. Shoulder pain , muscle pain in shoulder has supraspinatus tear
Recently diagnosed Blood pressure and has been put on blood pressure tablet from past 3 months
Dilzem SR 90mg
She has been having high pulse rate of 100 at resting and at times goes till 120 also.
Current medication
1. Delizem SR 90 --- Blood pressure and pulse
2. Rozavel F --- cholesterol
3. Budecort 200 MCG - Asthma
Current problem :
My mother has been diagnosed with Bronchitis asthma in 2015, she use to take sereflo 125
But she stopped taking inhaler from past 7 – 8 months as on taking inhaler she was feeling high palpitations
Recently she was getting little breathlessness and was snoring a lot in the night with cough and as an observer I could observe her sleeping patterns like she snors and then skips 2 or 3 breaths the noise goes away and then again snoring starts .
Basis the PFT report which we did recently doctor asked to continue the inhaler
But my mom felt very high palpitations for around 3 hours after she took the puff and we had to rush to the casualty yesterday night becoz her pulse went up to 120 and palpitations were not stopping In the emergency, docs did the Cardiac profile test , her pulse came as 118 which the doc said its high but je said may be it is due to the puff the palpitations have started
The doctor changed the inhaler today to Budecort 200mcg
current complain : Even after taking Pulse rate and hypertension tablet her pulse rate goes till 110, 120 at times and blood pressure keeps on fluctuating it comes to 155 over 120 and 52 over 80 .
Questions
1. Please examine the Ambulatory test report done recently and help us understand the reason of fluctuating Blood pressure.
Please advice further on the investigations to be done and the medications .
I am really very concerned as her blood pressure has been on the higher side always from past 5 months and medications have been changed but still no effect
I am attaching the latest Echo report, ECG report, TMT report and Ambulator bp report . please help evaluate the same.

Her Echo and TMT reports are essentially normal for her age. I’ve reviewed her ambulatory BP report and that is also overall all right except for one or two readings of slightly high Blood pressure.
I would like to tell you that blood pressure is an ever changing phenomena and can’t remain constant every hour as it depends on our activity. Emotional state, stress levels and other physiological phenomena.
Her tachycardia is because she has anemia so the body is trying to compensate so as to supply proper oxygen to all body parts by increasing heart rate slightly.

AS per the Ambulatory BP test her BP is quite fluctuating like it reaches to 160 and the daistolic pressure goes down in the range of 50's. Also my mom gets chest pain which goes to the back at time like once in 10 days and all, doc said it is gastric problem and asked her to start nexpro 40mg capsule and pregaba 75mg tablet for 14 days
Also for Bood pressure he has added one tablet in the night called as Telma 40 mg once a day affter dinner . My concern is she is currently taking delizem CR 90mg once a day post breakfast . Her BP is still not under control it goes to 160 and most of the time its 140 over 120 . Looking at the same doc asked to add one more tablet named telma 40mg . But I am concerned If I add the tablet telma 40mg the diastolic pressure is anyways on a lower side like in 50'2 it will go down more and cause a sever problem

Telma will not put the diastolic pressure down? As it has gone down to 52 and in 50's . I am worried about that if that gies down she might collapse and can you help me understanding the reason for high pulse rate even after talking the medicine Delzem SR 90 the pulse rate has not gone down.

But her lungs capacity has gone down and doc has asked to start seroflo or symbicort to improve the lung function.

And her body started shivering hands specially. We are worried as we have to take seroflo also becoz of lungs .

Serolo has this effect on her even earlier it was the same. So do you suggest she should continue with seroflo even if the heart rate goes to 115 120 types ?
Also, at times it is not possible that she goes to the hipsital to do ECG at the time of palpitation . So can she use the inhaler in the hospital only and immediately do the ECG ?

Just a last question on blood pressure and pulse rate. I understand beta blockers should not be given to asthma pateints. But can she stop dilzem sr 90 mg and instead take nebixart 2.5mg one in the night.
