It seems like an infected blister, along with inflammation of the lower leg and surrounding region.
As of now clean the area with soap and water and apply over the counter available topical antibiotics like mupirocin.
It is better to start on oral antibiotics, after consulting a general practitioner to prevent complications and further spread.
If there is pain, you can use analgesics/anti inflammatory combination like aceclofenac/seratiopeptdase for symptomatic relief.
