What Should I Do About Tender Pain In The Back And Elevated Creatinine Levels While On Diabetes And Hypertension Medication?

Question: I am a Diabetic patient for last 24 years. Presently for last 6 to 8 months my sugar levels are totally in control, on account of medicines I am on. However today when I checked my Creatinine it was found to be 4.4 . My fasting sugar was 90 but my hemoglobin is 8.5.

Six months ago I used to have swellings in both my feet upto thigh and many times on both my hands. These have disappeared since last three months.

I feel weak and cannot walk even 200 ft. I get tender pain in the back. Otherwise I am actively attending my office etc. Besides, sugar and blood pressure medicine I am on " dytor 40 tabs" . Please suggest how best I can reduce my creatinine levels and what precautions I should take now.
