Technique: Multiplanar, Multiecho, Axial, Coronal and Sagittal, T1 and T2 - weighted images without and wit contrast:

Report: Expansile lesion is seen involving half of mid brain returning hypointense on T1, hyper-intense on T2 and T2 FLAIR with heterogenous enhancement on post contrast images. Mass is compressing right half of mid brain, cerebral aqueduct with expansion and bulging of left half. Resultant prominence of lateral and third ventricle is also noted.

Impression: Gliomatous lesion involving left half of mid brain causing its expansion with resultant compression of cerebral aqueduct and prominence of lateral and third ventricle. Histopathological correlation.

For the Patient, based on the information given, the approach I would prefer is

1. Right VP Shunt : This is a procedure to reduce the pressure in the brain

2. Stereotaxy Biopsy --> Histopathological Correlation

Further treatment would be based on the results of Histopathology
