What Would Cause Painful Swelling In The Right Cheek, Under The Ear And Right Eye For About Two Weeks?

Question: Hello,

My right cheek, under the ear and the right eyes are swollen and causing pain for couple of weeks now. Can you please advise what could be the reason and possible solution? Thanks

Hello,

Pain and swelling of the cheek and eye can occur in sinus infection(sinusitis).
However, one important consideration that needs to be ruled out is infection occurring around the eye, called periorbital infection. Both sinus and periorbital infection can co-exist in some cases. These conditions require management with antibiotics as soon as possible to avoid complications. So please be advised to follow up with your primary provider at the earliest. You can take Advil 400 mg thrice daily for pain.

Could this be parotitis? I have some pain under the ear and eustachian tube area. I have no phlegm or cough. Slight nasal blocks, not always. The pain and swelling around the right cheek, near and around the eyes, near right temple area, under the ear and eustachian tube area are important to notice. I don't have pain behind the ear or inside the ear. Thanks.

Since you are complaining of swelling around the eye and temple, it is not very typical of parotitis. Parotid swelling tends to be closer to ear in the lower part of the cheek.

However, parotitis cannot be completely ruled out from this, so a physical examination is required for confirmation of diagnosis. So please be advised to follow up with your primary provider.

I have uploaded some pictures. If you can see. Does that look like sinusitis to you? Does infection in these area can be treated with Clindamycin 300mg 4 times a day? Thanks.

Possible diagnosis: Parotitis

Hello,

Thank you for attaching the pictures. It defenitely gives better idea about the location of swelling and pain. And it looks like parotitis rather than sinusitis. Parotitis can be confirmed by serum amylase testing.An ultrasound of gland may also be needed. Glucose testing is required to rule out diabetes. Treatment depend on underlying cause. If the gland is infected with bacteria (which is confirmed by gram staining the gland discharge), then clindamycin can be used.

Can i take prednisolone for a week to reduce swelling fast? Each time I take prednisolone, within a day or two, I get swollen feet for fluid retention most likely. Should I be worried if I take prednisolone for only a week? Thanks.

It is not advisable to get prednisone without identifying the underlying cause and prednisone is a medication that can be bought with prescription only. Taking prednisone for a week usually does not cause any issues. However, if there is underlying bacterial/viral infection it may worsen with prednisone. Please follow up with your primary care provider appropriate management.
