Question : What would you do if your patient texted you that they heard God talk to them? This happened right after a pretty even keeled session. The patient does have a psychiatric history of hearing voices outside of his head, but it's not a natural occurrence (no schizophrenia). Patient on new med. Mitrazipine for sleeping.

I read your query and understand your concerns.

First of all I need to state that the patients experience are not normal and definitely it is indicative of auditory hallucinations.

Since we understood the phenomenon now we need to evaluate the patient for possible causes. I understand that schizophrenia is first differential but it is not exclusive and there are variety of disorders which can present with auditory hallucinations. The prominent include mood disorders, neurodegenerative disorders (Age 59), alcoholic hallucinosis, substance abuse among others.

So definitely there is need for proper evaluation before you can pronounce possible cause for hearing voices.

In case there is no history of substance or alcohol abuse we need to look for other causes such as thought disorders, mood disorders, neurodegenerative disorders, idiopathic cause among others.

In my opinion brain scanning should be the first thing provided the hallucinations are of recent onset and does no present since very young age.
