First of all, medicine is not mathematics to get accurate answers. Here we can give the probabilities and the possibilities only that too sometimes in range.
You had protected exposures both vaginal and oral. Condom broken was not difficult to notice. When there is no obvious leak in the condom you need not worry about the other possibility.
I saw the pictures sent by you. All the symptoms explained by you are not specific only to HIV infection and can occur to anybody at any point of time without HIV infection.
Regarding your tests, Window period for HIV infection to detect antibodies against HIV is around 6-8 weeks and may be delayed up to 12 weeks.
Your 4th generation test has two parts:
1.Antigen detecting part: Only detects HIV 1 infection and not HIV 2. It becomes more conclusive after 3 weeks.
2. Antibody detection part can detect or rule out both HIV 1 & 2, but will become more conclusive only after 12 weeks.
Regarding HIV Qualitative RNA PCR, it can detect the presence of HIV 1 infection only and not HIV 2 which is very rare to get-less than 0.5% prevalence in countries other than certain parts of XXXXXXX. By 16 days after the exposure, this test is reliable to a considerable extent.

1- Dear Dr, can you please tell me how accurate was my RNA test done at 16 days (in percentage please because numbers make me feel safer and they give me a better vision and understanding about the possibilities)?
2- Dear Dr, honestly, is there any need to repeat any test in the future to rule out the possibility of being infected with that last exposure back in December? Even if there is a 0.001% need or doubts, please tell me. I am currently in my 12th week post exposure.
3- Dear Dr, based on your long professional experience and based on the researches you have read, have you ever seen or heard about a negative RNA test done at the 14th OR the 15th OR the 16th day post exposure turn out to be a false negative and as a result end up with a positive result at the 12th week with an antibody test?
4- Dear Dr, what makes doctors here tell absolutely different opinions when they were asked about the accuracy of the RNA test? all of these doctors are here on this website and all of them are certified doctors and this is science and science is a really solid subject. Another negative HIV test will relieve unnecessary mental tension

Hi,
Your negative qualitative HIV RNA PCR on day 16 after your encounter is in my opinion more than 90% reliable.
As you had a disturbed mind with regard to your condom whether broken or not and disturbed by the symptoms, it is better to have one more 4th generation HIV Combo test 12 weeks after the exposure and rule out the negligible possibility. You will get relieved mentally. Otherwise, in future, you will be likely to correlate any head ache or body pain to the past episode and worry.
I have not seen or heard of anybody's negative HIV RNA PCR 16 days after the exposure become positive for HIV at 12 weeks after the exposure.
Regarding the interpretation of test reports will be on the basis of the various physician's apprehension about the disease, of course and behavior of these tests.

If your statement regarding protected sex during both oral and vaginal sex is absolutely correct, then having one other test is just to relieve your mental tension and not to double check.
Have you used separate condoms for oral and vaginal sex?

Hi,
Condom is meant for use and disposal. One condom for each use whether it is vaginal, anal or oral.
Have you removed and reapplied or was the condom in place without being removed during both acts? Have you not ejaculated during oral coitus? This type of lubrication with saliva before vaginal sex had a predisposition for slipping or breaking. Anyhow, if you have used two separate condom for each act, your protection was complete and the risk is almost nil.
