When Is IgG Immunogobulins Count Considered High Enough For Multiple Myeloma?

Question: Yes Which hospital is best for the treatment of Multiple Myeloma? I have IgG Kappa light chains. Serum IgG immunogobulins of about 2800. No Bence XXXXXXX in Urine. Beta 2 is about 2.8. Below normal red blood cell count. Hemoglobin at low point of normal. I have a lot of leg pain below the knee which is relatively new for me. I can't tell if it is the bone, nerves, or muscle causing pain. Diagnosed with MGUS in 2009. Older brother diagnosed with multiple myeloma in 2011. Paternal grandmother had leukemia and maternal uncle did too. At what number of IgG immunogobulins will be considered high enough for multiple myeloma. How high does Beta 2 have to be? I had to have my parathyroids removed and I take calcium supplements and D3 so I do not know if calcium levels will show accurate destruction of bones. I also have a goiter with a couple of growths on my thyroid that are being checked every 3 years. I have hypothyroiditis. My lymph nodes in chest are enlarged but benign. I have had a fundic gland growth removed from my stomach two years ago and will be going back soon to have a second one removed if possible. Lucky me. I have cardiomyopathy with congestive heart failure, acid reflux, IBS, high blood pressure, sleep apnea which is untreated, ect. I have thought about going to Mayo Clinic and letting the doctors there put their heads together and see what they come up with. I am a 60 year old white woman. I have had joint pain for many years and have degenerative back disease. I am overweight. It is hard to exercise with joint pain. I just had a bi-ventricular pacemaker replaced and I am in a weakened state because a lead to my pacemaker has been breaking down over the last two years. A new lead was put in. I guess I am just scared. My hematologist says I will not die from multiple myeloma. My heart will kill me. My cardiologist says I will live another 20 years. Are these doctors just blowing smoke to keep me from worrying? I have not been the luckiest person in the world! Let me just add that I have been having fainting spells off and on since the age of 14. Sometimes I loose consciousness and sometimes I don't. When it is bad, I fall a lot. Six falls last year. None so far this year. MRI on brain 20 years ago just showed chronic sinus disease. No doctor has been able to identify the cause for the fainting spells. I have also lost over half my hair on my head and all the hair under my arms over the last few years. My added weight seems to be mostly in my neck and trunk.

Brief Answer: Multiple Myeloma Detailed Answer: Dear XXXX I am sorry to find that you are having multiple problems in your body and I while sharing my sympathies with you have to say that out of all these problems multiple myeloma is most worrying. Beta 2 is a prognostic marker. IgG higher than 3000 is multiple myeloma and diagnosis is confirmed with a biopsy. Hypothyroidism and hypo parathyroidism complicate the problem. You need to be under the directions of a multspecialty centre where all the problems can be tackled under one roof. When multiple myeloma is under control many problems in the body will be addressed. Cardiomyopathy , congestive cardiac faliure are chronic problems but it is multiple myeloma which if not in remission is a serious threat. Many variety of drugs for the same are available which a good Oncological centre like Mayo Clinic will prerscribe. While getting treated for these problems I advise you to keep you morale up which will help you to endure the problems and help you in responding to the treatment. I wish you good luck. Truly, Dr. J.
