When Should Be Anti-rabies Vaccine Shots Be Taken?

Question: Hello Doctors, I have a question about Verorab vaccine boosters. Approx. 4.5 years ago I completed a 5 shot post-exposure series of Verorab following a bat exposure. I have since had boosters due to minor exposures and the last 2 shots were about 13 months ago. Now this week I was nibbled by a street dog in Thailand, no blood or scratches but a small red mark on my arm. Taking no chances I went to the emergency room for booster shots and received Verorab on monday evening around 8 pm. I know the schedule is D0 and D3 so I went for my second shot at thursday morning at around 9am. My question is if I went for the second shot too early? Does it make a difference if the first shot is taken in the evening and the second in the morning? I went on D0 and D3 but there was 61 hrs between shots not 72 hrs which would be exactly 3 days. Does the 11 hrs make a difference? Your kind expertise on the matter would be greatly appreciated.

Brief Answer: You are completely covered against RABIES Detailed Answer: Hello, I am Dr.Omer and I am here to help you with your query. I am happy to know that you know alot about anti -rabies vaccination.You had multiple exposures with animals including dogs and bats , but had a D0+D3 post exposure treatment as-well and not to forget about the booster doses you had annually. A person who has completed the 5 dose of anti-rabies vaccine have a antibody titer of >0.5IU/ml and usually sufficient for 20 years if you dont get a second lick or bite. This time you had a lick only with redness on arm which makes you Type 1 category and usually treatment is not required according to international guidelines.But no risk should be taken when you have any kind of animal exposure from mild to severe as rabies has not treatment and for Type 1 category i do suggest rabies vaccination. The formula made for re-exposure of rabies is D0+D3 rabies vaccination , with a delay of 72 hrs but plus/minus 15 hrs in second dose is considered totally normal.The reason for delay of 72 hrs in second shot is : 1. To see any adverse effects from first injection. 2. To let the body make antibodies more and more till the second injection, as there is a dip in antibody formation if second injection is given after 24 hrs according to studies. In thialand verorab also comes in intradermal preparation ( skin) but that is not that effective than verorab ( muscular injection) , just check this. In my expert opinion as i have treated so many patients locally and internationally i would say that your history of multiple rabies vaccine will protect you completely and the new rabies vaccine which you got this time is completely potent even if you have received the second injection 11-15 hrs before the time suggested. Even with all this explanation if you are not satisfied that you are completely protected from rabies , get a anti rabies antibody titer that will confirm you what i said. So stay happy and do visit Phuket beaches and dont take any tension that you have received rabies second shot 11 hrs ago. Thank you for your kind answer DR. Omer, The vaccinations were given in the upper arm deltoid muscle. just a few follow-up questions if you don't mind: 1. Just to confirm, in your expert opinion 61 hrs. between first and second dose is sufficient for vaccine to have full potency? 2. When is the earliest I can take an antibody titer test and what number should I expect to be protected? Thanks again!

Brief Answer: You are healthy individual with rabies protection Detailed Answer: Hello again XXXX, Thankyou for understanding. So this means you got intramuscular vaccine which is more potent. Yes, deviation from rabies schedule is consider normal with a period of 24-48 hrs either early or delay between two doses but not more than that. An adequate antibody response is seen in 7-14 days after completion of rabies vaccination, so the test should be done after this period.Complete virus neutralization at a 1:5 serum dilution when determined by rapid fluorescent-focus inhibition test (RFFIT). WHO states that an antirabies antibody titer of ≥0.5 international units/mL or 1:5 serum dilution can be considered protective. This serological testing of rabies is not indicated for you as your immunity is good , you used international brand of rabies vaccine , deviation from rabies vaccination schedule is <48 hrs and you are not a veterinary doctor.
