It is very unlikely that you could have had chronic (long-standing) epididymitis without having any symptoms of it. Men with chronic epididymitis generally present with a dull-aching scrotal pain. Many patients also have associated testicular pain. Sometimes, patients can present with localized pain or discomfort in the epididymis alone. Other associated symptoms can be painful urination, fever, enlarged lymph nodes, urethral discharge, painful intercourse or ejaculation, etc. Even in cases of acute infection, symptoms should evolve within a matter of a few days maximum. So, it is very unlikely that you could have had inflamation / infection of the epididymis for a long time without having any symptoms and present with pain all of a sudden.
