Why I Feel Incidentally Of Extreme Anger With Suicidal Thoughts At Times?

Question: Hello Sir/Madame,

My friend have been suffering from the following issues since last few months:
1. Tiredness
2. Difficulty falling asleep
3. Highly anxious if any Noise disturbs in either falling asleep and or any Noise wakes him up.
4. Mild difficulty concentrating.
5. Talks a lot with real facts and incidents but with negative ir victimized thought around it.
6. Incidentally of extreme anger with suicidal thoughts at times.
7. A continuous feeling of being exploited (due to recent real incidents in his life)


His condition started deteriorating when All XXXXXXX level exam got cancelled and repeatedly postponed due to Covid 19 situation. He had been preparing for the exam since years.
He is able to remember all instances in life. His normal daily routine involves the following:
- Wake up at around 10am
- Freshen up and have Tea/Biscuit
- Watch TV and play football for sometime
- Have lunch
- Try going to sleep around 7pm

But, he gets extremely aggressive and after continuously talking about all the past negative experiences if any noise affects him while he is trying to get asleep or he us walked up by any noise.

We have recently shifted to a local hospital and he is being treated with the following medications here:

-Oleanz
-Sizodon
-Dicorate
-Sompraz D40

The exam for which ge was preparing is finally scheduled (after a delay of 1year) on 25th XXXXXXX 2021. He is continously insisting to go out from the hospital so that he can study and effectively appear for the exam. He is also insisting that he would continue with the medications while studying in home.

It will be helpful if you can kindly suggest on the following:
- Whether any change in medication is required - if he would be able to efficiently appear for the exam with the effects of medicines.

I read your query and understand your concerns.

First of all I feel we need to focus on his health rather than career at this time. If the health is excellent definitely we can achieve better career options.

Secondly I do not feel medication can be changed without seeing a patient. There are number of situations which can be considered by treating doctor only. However I feel number of antipsychotics can be decreased after due consideration of his condition. As per the available details he is receiving two antipsychotics at present (Oleanz and Sizodon) but can be managed with either one as most patients respond to one antipsychotics.

In regard to your question about possibility of attending exam I feel he can definitely appear for the exam but the efficiency can be in question as the disease as well as medications affect the higher mental functions.
