Question: I have a fever that comes and goes for several months. I had blood work done and it looks pretty normal, the only thing it says is slightly elevated is monocytes at 8.2. I know a fever for no reason is not normal. I want to figure out what’s going on, any suggestions?

Brief Answer:
Many causes of monocytosis

Detailed Answer:
Hi
Thanks for your query.

Absolute monocyte count upto 800 per microliter is normal. Do you mean the level is 820 per microliter? Because 8.2 X 1000 per microliter is too high. 820 is not that much. Or do you mean 8.2% of total white cells? That's also not a great deal.

Fever with monocytosis can be because of number of reasons. We need to know the fever intensity and pattern. What's the XXXXXXX temperature recorded ? Does it come daily?

Any associated symptoms like weight loss or night sweats?

In general fever with monocytosis can be due to various causes like chronic infection such as tuberculosis, endocarditis, granulomatous disease like sarcoid, or in bone marrow proliferative disorders. Also some autoimmune disorders including rheumatoid arthritis.

We need to examine the blood film for any abnormal cells and also do CT scans of chest and abdomen, in addition to cultures and other microbiological tests for infections.

Hope this helps.
Regards
