Question: Why am I getting difficulty in breathing and a heavy feeling in the chest?

Hi there,

The last year 2019 in July I was traveling on a public bus from my office to home at 7 PM. I got breath difficulty and some heavy feeling in the chest area. I immediately went to one Cardiology Hospital and joined in the emergency ward. They have run all necessary tests (ECG, 2d echo, etc.,) and told nothing problem with the heart and discharged me from the hospital that night itself.

But my problem remains, I struggled with the same problem day and night every day. I feel I am going to die with breathing difficulty. Later I went to 2 more Cardiology Hospitals and done all tests like ECG, 2d echo, TMT, complete blood profile. Again the same answer I have no health issue.

But my problem remains like that only. I get breath difficulty and heavy feeling in-cheat daily. I get breath difficulty irrespective of my activity like while walking, sleeping, talking, sitting all the time.
Later I visited pulmonologist and he did a chest x-ray, PFT test. He said I have no issue with my lunges.

But my problem remains, I very much scared about my life and approached life insurance company to take the policy. They also did all kinds of tests to me before giving the life insurance policy. They have approved my policy too.
I have visited a psychologist to check if I am getting breathing difficulty due to my anxiety. They have taken my through some anxiety questionary test and confirmed I have no anxiety.

But still, I am getting the same breathing difficulty and heavy feeling chest. I am getting mad with the question that why I am getting this problem and whom should I approach?
Please help me by answering. I am struggling with this problem for around 10 months.

My lifestyles
I am male. Age 32. Weight 56 Kgs. Hight 5.6 feet.
I am a non-vegetarian. (I eat seafood, chicken, and mutton)
I used to smoke and drink but I left it 2 years ago completely.
I am a Digital Marketing guy. I work 5 days a week.
I got married one year ago.
I have no other health issues. I did not admit to any hospital from last 20 years.
I don’t have a cough.

I do feel your pain as in spite of all the tests being normal, The problem still persists.

Going through your query I presume that all heart and lung-related tests are done and they are normal, though I would be happy if you can upload so I can help interpret it for you.

So, two differential I can think of based on the history you have provided.

1) GERD/Reflux Gastritis- Here the stomach acid reflexes back into the food pipe causing burning chest pain and breathing difficulty. A review with a Gastroenterologist to rule out GERD / Hiatus hernia is what I suggest or a trial of plain antacid for 6 weeks.

2) Panic attack:
The 1st episode happened on a crowded bus.
A new marriage, Job stress both would result in our subconscious breaking down in a particularly stressful situation.

You can carry a tablet. clonazepam 0.5 mg in sublingual form. during the next episode put one beneath your tongue and try relaxing (this should be tried only after consultation with your doctor).

You are young and with all basic tests normal, nothing is grossly wrong in your body. So find relaxation techniques whichever helps. You would do just fine.

Reports contain 2D ECHO Cardiogram - Doppler Report, Complete Blood Cell Count, ECG, LIPID PROFILE, Lungs X-Ray, PFT, and TMT.

You have expressed 2 possible causes.

1. GERD/Reflux Gastritis - If it is happening because of gastric problems, will it be confirmed by conducting an upper endoscopy test? or suggest to me what kind of test that I need to go through to find the root cause.

(One small addition: Without the test, I assumed maybe I am getting problem because of the gastric problem and I used pantocid dsr, zincovit for 3 weeks but I see no results so I stopped using it)

2. Panic attack: My doubt is if it is a panic attack should it come in particular conditions? Because I am feeling little chest pain and breathing difficulty daily from the past 10 months without a single day gap. I am not having any stress in my personal and professional life, anyway I try meditation to overcome this if I am feeling some kind of stress inside.

1) Yes upper GI scope would help the diagnosis.
2) Take only antacid .
