Why Am I Having Right-sided Abdominal Pain That Is Worsening Over Time Along With Nausea?

Question: Hey doctor for the last 48 hours now I have been having this ache like pain on the right side of my abdomen that seems to be getting worse over time, I have attached an image of the exact location. I have also had bouts of stomach pain in the centre along with nausea. But it mainly feels like a consistent type of ache the right side that sometimes turns into a sharp pain. I went to see my doctor about it and he said it might be a stomach bug since I recently traveled to the US from Australia but I am unsure if that is right. What else could it be I was tested for gallstones in August last year and I have a family history of gallbladder problems and fatty liver mainly with my Mum having it and I don’t think it would be my appendix since that’s lower down right?

Acute appendicitis is unlikely though retrocecal appendicitis can present even with epigastric pain so the position/ area involving the pain alone is not important. With appendicitis, there is associated guarding , rebound tenderness and rigidity. The pain is intense. There is a high white cell count, too. The abdomen needs to be examined, in any case.

Based on your description , the cause for the pain needs to be determined. Ultrasound of the abdomen and pelvis, urinalysis and stool routine exam plus exam for ova and parasites should be advised. Viral hepatitis profile, basic metabolic panel including liver function tests should also be done to look for the cause.

For now if the pain is vague and not intense with no symptoms of intestinal obstruction then antispasmodic like alverine citrate in combination with simethicone can be given to look for improvement, in consultation with your doctor.
