Why Am I Prescribed Progesterone Tablets In Pregnancy?

Question: Hi Doctor, I am currently pregnant and my due date according to my scan is 22nd of Oct 2014. I have entered my second trimester, my doc has advised me to take folic acid and shellcal (calcium ) tablets and also continue to take the dosage of Duphaston and Sustain as was prescribed in my first trimester.

Mine was a case of delayed ovulation and my NT scan at 12 weeks was perfect , only thing of concern was that I was told to have a low placenta. I am also suggested to take the Qudraple Marker test in my 16th week.

My question is , why am I prescribed progesterone tablets? This is my first pregnancy and I am not feeling any spotting or abnormal discharge. I read that it might effect a fetus too?

Please advise me.

Thanks for writing to us with your health concern.
Progesterone tablets are prescribed in high risk pregnancies to avoid a risk of miscarrying or preterm labour.
In your case, the placenta is low lying.
It carries a risk of bleeding / spotting or miscarriage at any point of time.
Hence, it is better to take progesterone as it inhibits uterine contractions.
However, you do not need to take both Duphaston and Susten.
You can stop Duphaston and just continue to take Susten.
Also, once the second trimester starts, you should start taking Iron and Calcium + Vitamin D supplements also.

Thanks for writing back Doctor, should I take sustain vaginally or orally? Please also suggest some measures or lifestyle adjustments through which I can reduce my risk or miscarriage or preterm labour.

Susten taken vaginally has better absorption.

To minimize risk of preterm labour / miscarriage -
1. Sleep properly. Atleast 8 hours in the night and 2 hours nap in the daytime.
2. Take lots of water, to avoid dehydration and risk of urinary tract infection.
3. Avoid stress.
4. Avoid hectic work, lifting weights , bending, heavy exercise, prolonged periods of standing or walking.
5. Avoid intercourse till your doctor permits.
6. While inserting Susten vaginally, atleast 2 finger breadths of the finger should be inside.
7. Keep meticulous perineal hygiene. Wash with lukewarm water over your private parts after using the washroom. After a bowel movement, wipe from front to back, never back to front.
8. Do not take any drugs / antibiotics without consulting your doctor.
