Question : Hello doctor, i have this long standing issue which is that i don't want to speak or talk much to anyone. I mean i feel like heavy from deep inside to speak at continuation or just to speak anyone on phone or in person. The main issue what i feel is not wanting to speak but rather it's more like some physical issue with me, that makes me to not speak much. I get feeling of like heavy burden inside me if i have to or while speaking to anyone, i like to stay quiet with minimum interaction.

What's exactly is wrong with me?

Although it is not possible to make a proper diagnosis with available information I feel it could be due to anxiety disorder or related anxiety spectrum disorder.

The other possibility is social anxiety disorder which again a form of anxiety disorder.

I request you to see a psychiatrist in person for detailed assessment, diagnosis and treatment.

I did actually consulted doctor, i do have anxiety disorder along with frequent mood swing issue so told to take sertraline 25mg as start but the issue is i came to know that it may cause male fertility issues such as sperm defects etc? how much true or chances of causing that?

The chances of causing sperm defect is not conclusively proved and there is no data to support such claims. As per the available literature there is no chances of causing male infertility with use of anti anxiety medications.
