Here are the possible causes for your condition:

1) Heart arrhythmia which causes a decreased cardiac output and syncopal episode. I would suggest a Holter monitoring to rule out paroxysmal arrhythmias.

2) Orthostatic hypotension

3) Benign paroxysmal positional vertigo

4) Situational syncope.

We can perform a Diller hall pike maneouver test to rule bening paroxysma positional vertigo. This is done by ENT doctor.

If your holter recording has shown episodes of fast heart rate we need to know the cause for the same. Atenolol is the right drug for a fast heart rate. You can have a thyroid level checked to make sure you don't have hyperthyroidism. That can cause paroxysmal tachycardia.

I would suggest MRI brain to rule out a vertibro-basilar insufficiency. If the circulation to the posterior circulation of the brain is compromised, this can lead to a similar condition.

Also the kidney has a limit to which it can deal with excess sodium and fluid load. If this is exceeded, it can lead to high BP. In your case you are on treatment and have good kidney function hence the brooth did not change your BP.
