As per your symptoms, the possibility of intrascrotal or testicular pathology is high.
It's advisable to rule out testicular malignancy in view of your age and harder consistency of the swelling.
For this it's advisable to get a scrotal ultrasound Doppler at the earliest to rule out malignancy
