Brief Answer:
CT scan and EMG test

Detailed Answer:

Hi,

After hearing your history I would advise you to do a CT scan of brain, EMG test, 2D echocardiography and angiography test. The exact cause of your symptoms needs to be found according which treatment can be planned.
