Why Would A Person Have Tinnitus For Over Two Years After Surviving Breast Cancer?

We have consulted the ENT doctor again with MRI report and she was not able to tell us the reason for her problem. She also told audiometry is not needed as she don’t have any hearing loss.
She just gave Mactotal for 3 months and I am not sure how this Vitamin capsule will help in fixing the problem.
We have also consulted the radiologist who did the MRI to get his opinion again and he is absolutely sure that there is nothing concerning in right side and even the one mentioned in left side can be ignored as it is not a major finding according to him.
Kindly suggest what can be done next as she is suffering from this issue almost 2+ years now.
Details as discussed below:
Detailed Answer:
Hi,
1. Her MRI Report is within normal limits for her age.
2. Upload a latest Audiogram, Tympanogram (Impedance Audiogram) and any Tinnitus frequency matching if performed. Tinnitus can occur independent of any hearing loss.
3. Ask her to rule out pulsatile (vascular) tinnitus by checking the rhythm of the sounds as compared to her heart (pulse) rate. Changes in tinnitus with clenching of the jaw and on head / neck position / pressure indicate other causes as compared to true Cochlear synaptic tinnitus.
4. I have noticed patients who have increased Tinnitus when they are stressed, have fluctuations of blood pressure, low hemoglobin, low blood sugar levels and so on. All these conditions must be ruled out.
5. Cochlear Synaptic Tinnitus may be treated in up to 50% people with Caroverine (Tinnex) infusion followed by capsules for two months. Other treatments such as transtympanic steroids, HBOT (Hyperbaric Oxygen Therapy), gingko biloba, multi-vitamins and anti-oxidants may also help in reducing Tinnitus. TRT (Tinnitus Re-training Therapy) with a new tinnitus treatment software is possible.
5. Kindly let me know the results of your Audiogram. This will help in suggesting further treatment.
6. I must emphasize that in a vast majority of patients of tinnitus, the cause is never found. However there are further treatment options depending on the type of tinnitus.
I hope that I have answered your query. If you have any further questions, I will be available to answer them.
