Question : I am a 59 year old female with a history of heart disease, high blood pressure, high cholesterol, and type II diabetic as well as overweight. Quit smoking the day of my heart attack on 12/30/2010. On 2/30/2010 I had a treadmill stress test along with lexiscan. I was sent home even though they saw problems. Within 30 minutes of getting home, I suffered the widow maker. (1 stent in LAD). Since that time in Aug 2011 I had a right femoral endartectomy. Over the last 6 months or so, I have had terrible leg pain when walking so cardiologist ordered CT Scan of abdomen and legs which showed several blockages in both legs and both iliac arteries. Before sending me back to the vascular surgeon, he has to do a stress test and then a peripheral angiogram. However, if I cannot reach my target heart rate they will have to use Lexiscan and I am scared to death of that causing me to have another heart attack as it has a bad rep along with the FDA warnings about the drug that came out in 2013. Do you feel it will be safe for me again? What are the chances of it causing another heart attack or worse? I am petrified. I told Dr of my fears and he says I will be fine.

After going through all your medical reports as well as medical details provided by you carefully I would like to assure you and tell you that I agree with your treating doctor that you will be all fine. The reason of my assurance to you is that your heart function has dramatically improved from 35% to more than 50% which is actually very good and above that you have stopped smoking also.

The reason of not being worried with the use of lexis can this time is that your coronaries have been evaluated now and what ever blockage was there is already been opened up by a stent. Last time they were not knowing about the blockage that's why it caused an attack.

So just relax, have faith in your doctor and most importantly be positive as you are going to be all fine.

Thank you so very much Dr. XXXXXXX partap for your reply and assurances that I will be fine. I do understand that in 2010 they did not know of the blockage and that makes sense to me, but what if there is something now that they do not know about? I had the stress test in 2015 and it was good. Can arteries get blocked that quickly? I will do my best to take your advice and try not to worry and let your words along with my Dr's words boost my confidence. I guess the main fear again it from what happened in 2010.

Ma'am the rate of development of plaques inside the vessels is a relatively slow process. So it's highly unlikely for a clinically significant blockage to get developed over this small period of time.

The process becomes even more slow as soon as the patient stops smoking ( As smoking promotes atherosclerosis in the vessels).

Dear Dr. XXXXXXX Partap

I want to thank you again for all your information, support, and positivity. I had the stress test and was able to complete it without the use of lexiscan. I maintain through all the leg pain for 9 minutes and got it done. My doctor came to the stress lab for a preliminary look at my EKGs and images and said all looked good. The words you shared with me helped me to accomplish this.

The main reason they ordered the stress test was to evaluate my heart health for the vascular surgery that needs to be done on both legs and iliac arteries. I am hoping they can do them all at one time but I know we have to do what is best for my heart and me.

I love this service that is offered and will use it again in the future.
