Question : With the combination of these 2 reports, am I looking at surgery? I am raging in pain right now using my medical marijuana that's barely touching the pain. My orthopedist doctor is not until April 4th I have to give him these results amongst others from 2012 14 and 17 for my right shoulder and my thras expinse and my right hip. I've had the epidauros with steroids I've had the blocks I've had the sarge therapy I've had acupuncture accu pressure and seds and I'm allergic to Pregnant zone the last time I had trigger point injections with pregnant zone my face broke out in a rash. I have been on Tylenol for 4 years and my liver enzymes are253... I have been disabled for 20 years from a knee injury when I was 27 years old... So far the knees still good I've had to offer scopic surgeries on that KNEe. I've had I believe the EMG where they put the wires on your head and see how far you could feel the electricity go down your leg? I'm not sure if that's the correct test the name. I carefully went through your query. I am sorry to hear that you are going through such a lot of pain and that seems to not be responding properly to the treatment.

In the reports of the shoulder it is noticed that there is a small tear in the cartilage within the shoulder joint which most of the time is not serious and nonsurgical treatment is tried first with a lot of effect in many patients and rarely the surgery is needed to repair it. With nonsurgical measures (painkillers and physiotherapy) this might take a couple of months to completely resolve.

As for the changes in the cervical spine, you do have spinal stenosis which causes pressure on the spine and might be responsible for the symptoms you are having as well. Even in this case the same nonsurgical treatment is tried and if no improvement or any worsening surgery might be considered.

On the other hand please be aware that surgery in the neck has its own risks and might cause damage to the spine and not necessarily relieve the symptoms so it is very important for your orthopedist or the neurosurgeon (normally in this case these two should work together) should do a thorough examination on you and balance well the pros and cons of having the surgery or not and make sure you have chances to benefit from it.

Some drugs to try and that might help, I am going to mention below. They are prescription drugs so I am not sure you will be able to have access but probably your PCP may prescribe them for you.

- pregabaline 50 mg times a day, it may be increased if needed in a week (this is for nerve pain)
- baclofen 5 mg 3 times a day (to help relax the muscles)
- high doses of Ibuprofen (800 mg 3 times a day after food combined with Omeprazole 40 mg 30 minutes before breakfast to protect the stomach)
- you may continue to use the marijuana with the above drugs

Seeing a physiotherapist is quite recommended to relieve the muscle spasms and help with pain so please make sure you do set an appointment. Physiotherapy is quite recommended for both the spinal stenosis and for the tear of the cartilage in the shoulder to help release pain and restore function.

Very likely the test you mention above is both EMG and NCT (for the muscles and the nerves) as these are generally done at the same time.
